** The 8 1/2″ x 11″ certificate is a legal, official document.
** It includes the individual’s name, date and place of birth, as well as the name and birthplace of the parent(s).
** The Governor and the State Registrar sign each heirloom certificate.
** Purchasing an Heirloom Birth Certificate helps to protect Washington’s most valuable resource – children.
** More than half of the certificate’s cost benefits the Children’s Trust Fund of Washington.
** For each $40 purchase of an Heirloom Birth Certificate, $25 is tax-deductible.

Fees for each certificate ordered: $20.00 US – (Make checks/money orders payable to Department of Health). We accept cash, checks, MasterCard and Visa.
Can I order a certificate for a birth, death, marriage or divorce that took place in another state?
No. We only have certificates for births, deaths, marriages or divorces that took place in Washington State. If you are currently living in Washington State but were born outside the state, you will need to contact the state you were born in. For events taking place in another state, contact the state where the birth, death, marriage or divorce took place.
I haven’t received the certificate I ordered. What should I do?
** Contact the agency or organization you ordered through. If you ordered through a county health department or another company (example: United States Vital Records), you will need to contact them.
