Corrections to Death Certificates
** The individual requesting a correction must include a legible copy of his/her valid government issued photo ID, such as state-issued driver’s license or non-driver photo ID verifying his/her current mailing address.

** Mail requests are processed in approximately 8 to 10 weeks from the date of receipt.
How To Correct Personal Information Of Decedent If Death Occurred Within Past Two Years :
** To correct spelling or typographical errors in the decedent’s name, date of birth, age, mailing address, social security number, etc.
** It will be necessary for the funeral director who signed the original death certificate or the informant listed on the death certificate to complete a Death Correction Statement.
** Please list the information in the “Correct” column and hand sign in the designated area.
** The individual requesting the correction must include a legible copy of his/her valid government issued photo ID
How To Correct Personal Information Of Decedent If Death Occurred More Than Two Years Ago:
** To correct spelling or typographical errors in the decedent’s name, date of birth, age, mailing address, social security number, etc.
** It will be necessary for the funeral director who signed the original death certificate or the informant listed on the death certificate to complete a Death Correction Statement.
** Please list the information in the “Correct” column and hand sign in the designated area.
** In addition, a photocopy of any one document that conclusively establishes the information to be corrected must be submitted.
** Acceptable documents include a birth certificate, Social Security card, marriage record, or life insurance policy.
** The individual requesting the correction must include a legible copy of his/her valid government issued photo ID.
Major Changes to Decedent’s Name, Date of Death or Cause of Death :
** It will be necessary to contact the physician or medical examiner/coroner who signed the original death certificate for assistance in changing these items on the death certificate
