Renewal Instructions :
** Renewing your license online is simple and efficient, taking only about 5 minutes.
** The process requires you to enter information from the renewal materials mailed to you by DOPL, verify and update your unique licensure information, answer a few questions, and pay your renewal fees.

** During the renewal process, you will be asked if you have completed or will complete all renewal requirements, including continuing education hours.
** You must also certify that you are qualified in all respects for the renewal or reinstatement of this license.
** Additionally, you will be asked to certify that you are the licensee identified in this renewal system.
** Therefore, this process should only be completed by the individual licensee – not by a secretary, office manager, or other representative.
** If the license is held by an entity, the individual most responsible for the license should complete the online process.
** Upon completion of the renewal process, you will be shown a Certificate of License Renewal that you can print and use as a receipt for your records.
** This certificate can be used as proof of licensure, allowing you to continue practicing until your renewed license arrives, via the U.S. postal service, in approximately 2 weeks.

** If you did not receive renewal materials in the mail, it is likely due to a change in your mailing address that was not submitted to DOPL.

** When renewing online, if you possess a secondary license (i.e. controlled substance), it will renew at the same time as your primary license.

Renewal Dates :
** Licenses renew once every two years, unless otherwise specified by statute.
** Each licensee is responsible to renew his/her license prior to the expiration date shown on the current license.
** Unlike many other states, Utah’s license renewal schedule is not based on the licensee’s date of initial licensure.
** Under Utah’s renewal system, all licenses in each occupation or profession expire as a group on the same day every two years.
** Therefore, the length of a licensee’s first renewal cycle depends on how far into the current renewal cycle initial licensure was obtained.
** Each renewal cycle thereafter is for a full two years.
** At least 60 days prior to the expiration date of a license, DOPL will mail a renewal notice to the licensee’s address of record as provided to DOPL by the individual licensee.
** Failure to notify DOPL of an address change could prevent a licensee from timely receiving renewal notification and other important correspondence from DOPL.
** New licenses issued during the last four months of an established renewal cycle will be licensed for a full two-year renewal period plus the period of time remaining in the current renewal cycle.
** However, renewal fees are not prorated.

Problems Logging Into The Renewal System :
** If you are getting an error which states, “Please check your login details and try again,” ensure you are doing the following –
License Number :
** Your license number can be located on your renewal form or from your blue license certificate.
** It is a six or seven-digit number followed by a hyphen and a four-digit extension: 123456-1234 or 1234567-1234.
When entering your license number :
** Be sure to enter the two parts of your license number in the separate boxes.
** DO NOT enter any spaces.
** If your license number begins with a “0” (zero), DO NOT enter the beginning “0.”
** For example, if your license number reads “0123456-1234,” only enter “123456-1234.”
** If you have an old license number that is preceded by an extra two digits, DO NOT include the beginning two digits.
** For example, if your license number reads “97-123456-1234,” only enter “123456-1234” and ignore the “97.”
** If you have a secondary license (i.e. controlled substance license), it will be renewed at the same time as your primary license.
** Simply use your primary license number when logging in and both licenses will be processed together.

Renewal ID Number :
** Your Renewal ID Number is located on your renewal form.
** If you did not receive a renewal form or if you have a generic renewal form, you need to contact DOPL directly to obtain your Renewal ID Number.

** A Renewal ID Number will never start with a zero.
** If the number that you have been given does start with zero, simply drop the zero when entering the Renewal ID Number into the system.
Social Security Number :
** If you are renewing a professional license for an individual, you are required to enter the last four numbers of your Social Security Number.
** Do not enter any hyphens or spaces – just the last four digits.
** If your license is for a business, company, or firm, leave the SSN field blank.
Problems Updating Your License Information :
** In attempting to update your license information, please realize that you can only update your address and telephone number online.
** Additionally, in submitting any change, you must submit all required fields – as indicated by a red asterisk ( * ).
** If your legal name has changed, you must verify the change by submitting a copy of a marriage certificate, divorce decree, court order, or social security card.
** Mail this documentation directly to DOPL.
** When asked to update your license information, you can only update your address and phone number online.
** A legal name change, education correction, or any other change to your license must be requested in person or by mail.
