ODH Apply/Renew/Amend an Ohio Radiologic License
** The Ohio Department of Health (ODH) now accepts online applications and payments for radiologic licenses.

** Applications are reviewed and approved within one to three days when received and paid for online versus two to three weeks if submitted through the mail.
** ODH recently enhanced the security of the X-ray Licensure System (XLS).
** Users are required to create an account in the ODH Gateway in order to apply, renew, amend or check the status of an application or license in the XLS.
** The ODH Gateway provides added protection to personal information.
New ODH Gateway Users
** If you have not created an account in the ODH Gateway, please click on the XLS Entry Form link below.
** You will need to have an e-mail address in order to receive your user ID and temporary password.
** You will be required to change your temporary password the first time you access the ODH Gateway.
** Additionally, you will be required to update the Security Question and Answer by selecting the Personal Info tab once in the ODH Gateway.
Existing Licensees and ODH Gateway Users :
** If you have an established ODH Gateway user ID and password, please click on the ODH Gateway link below in order to access the XLS.
** If you experience any problems with your user ID or password, please contact our licensure staff at (614) 752-4319, option 3, for assistance.
Hard Copy Application :
** Follow the link below to complete and submit your paper application.
** Please note that the application fee and all supporting documentation must be submitted with the application before it will be considered to be complete.
** Normal processing time for applications received via mail is approximately two to three weeks.
** ODH encourages all applicants to apply and pay for their license online.
Renewal Applications
** ODH will mail a renewal notice to each licensee approximately 60 days prior to the license expiration date.
** All renewal applications must be submitted online or by mail within 30 days after the license expiration date.
General Information & Fees
Radiologic License Requirements :
** Radiologic licenses are issued for the following categories – Radiographer, Nuclear Medicine Technologist, Radiation Therapist and General X-ray Machine Operator (GXMO).
** Please click on each link below for specific requirements for each category.
Radiologic License Fees :
Initial application fee: $65
Renewal application fee: $45
Current GXMO licensee converting to a Radiographer license: $15
Note :
** The Radiographer license will have the same expiration date as the current GXMO license.
** All fees are non-refundable and non-transferable; acknowledgement is affirmed when signing the “Statement of Declaration” section on the application.
Continuing Education :
** ODH now accepts distance learning for continuing education credits for all categories, specific to the license category held.
** The continuing education requirement is still 12 credits.
Examples of acceptable topics include :
** Quality control, quality assurance, or quality management practices;
** Principles of radiographic imaging;
** Radiographic, radiation therapy, nuclear medicine or bone densitometry equipment, instrumentation, positioning, or procedures; or
** Radiation safety and protection.
Examinations :
** RadEd was awarded the contract to develop and administer the GXMO exam.
** Exam applicants must submit the exam registration form directly to RadEd.
** Individuals may take the state exam any number of times.
** A GXMO who does not pass the state exam within one year of completing the didactic educational program must retake the didactic education prior to retesting.
Late Renewals
** Renewal applications received more than 30 days after the license expiration date must submit the following in order to be considered for another license –
** Application and $65.00 fee; unless the renewal did not occur due to an individual’s or spouse of an individual’s active service in the armed forces, in which case, the fee is forty-five(45) dollars;
** 24 CE credits;
** Evidence of completing the educational requirements (n/a for applicants in active status with ARRT or NMTCB); and
** Evidence of examination (ARRT or NMTCB card) – GXMO applicants must show evidence of passing the state examination.
Radiation Therapy CT Simulation :
** Radiographers or radiation therapists, who have obtained training and demonstrated competency with the CT equipment and protocols, may operate CT units for radiation therapy simulation procedures.
** CT simulation procedures shall be overseen by a radiation therapist.
** If the CT equipment is operated for any other diagnostic radiographic procedures, the CT equipment shall be operated by a radiographer.
