Death Records :
** Arkansas Vital Records has death records dating from February 1, 1914 through the present.
** The Arkansas History Commission keeps an alphabetical listing of deaths in Arkansas dating from 1914 through 1949, but they do not have the actual death records.

** All requests for certificates require photo identification.
** Please PRINT the name and address of the person who is to receive this request on the lines below.

What types of death documents are issued?
** Vital Records issues a certified copy of the original death certificate in paper form.

How much do death records cost?
** The cost of a certified death certificate is $10.00 for the first copy and $8.00 for each additional copy of the same record that is ordered at the same time.

** If no record is found or no copy is made, state law requires that we keep $10.00 for a search fee.
** You will receive a refund of any amount paid over the required $10.00 search fee.
** If you used the “Express Service” with a credit card, both the Express Service fee and the search fee of $10.00 are not refundable.

** You will receive a credit of any credit card fees paid in excess of the combined search and Express Service fee.
** We accept cash, checks, money orders made out to Arkansas Department of Health, and and all major credit cards, including American Express®, Discover®, MasterCard® or Visa®.
** Please do not send cash in the mail.

Who may order a death record?
** Arkansas Statute 20-18-305 allows release to specific persons who are related to the registrant, his/her designated representatives, academic research groups, and to persons who can demonstrate that they have a right to obtain the record.
** Death records that are more than 50 years old can be released to the public.

** Requester’s relationship to the deceased
** Requester’s reason for requesting the certificate

** You may order a certified copy of the death record by coming into this office.
** Orders are accepted for same day issuance from 8:00 A.M. until 4:00 P.M. Monday through Friday.
** The office is located at 4815 West Markham St. Little Rock, AR 72205.

Fees :
** 1st Copy costs $10.00
** Each additional copy costs $8.00

Note :
** Only Arkansas deaths are recorded in this office.
** There are only a limited number of death records filed in this office for deaths prior to February 1, 1914.
** The fee is $10.00 for the first certified copy requested and $8.00 for each additional certified copy of the record.

** The fee must accompany the application.
** Send check or money order payable to the Arkansas Department of Health.
** Do Not Send Cash.

** Of the total fee you send, $10.00 will be kept to cover search charges if no record of the death is found.
** Only the names and dates listed will be searched for the $10.00 fee.
** Names and other dates submitted later will require an additional $10.00 non-refundable fee.
** Mail this application, a copy of your photo id and the money to the address above.
** Please allow 4-6 weeks for processing.
