** Applicants must personally appear at the U.S. Embassy for all passport services.

Applicant is Under Age 16 :
** For children born in Eritrea applying for a first time passport, please see the link to the left “Report of Birth Abroad” for information on first applying for a Consular Report of Birth Abroad.
** The child and both parents need to appear in person unless one parent presents an original court order for sole custody, a death certificate for the other parent, or an original notarized DS-2029 – Application for Consular Report of Birth Abroad from the absent parent.
** Proof of parentage (original Eritrean birth certificate or original CRBA)
** Applicant’s most recent passport
** Parents’ valid passports or government-issued photo IDs
** DS-11 passport form
** 1 passport photo

Applicant is Over Age 16 and most recent passport is valid for 5 years :
** Applicant must appear in person
** Applicant’s most recent passport. If passport is expired, must present valid identification.

Applicant is Over Age 16 and Most Recent Passport is Valid for 10 Years :
** Applicant must appear in person

Most Recent Passport is Less Than 1 Year Old :
** Applicant (or child and one parent) must appear in person
** Applicant’s most recent passport
** If applicable: Evidence supporting change in identifying information (e.g., marriage certificate, amended birth certificate, court order, etc.).
** If applicable: “No Fee” letter provided when issued emergency passport

Fees :
** no charge if current passport is not a “no fee” passport or if data change is due to our error.
Lost / Stolen Passports :
** You may replace a lost or stolen passport by providing the documents listed below.
** Passports reported lost or stolen are invalidated and can no longer be used for travel.
** We highly recommend that you fill out the required forms online and print them before coming into the U.S. Embassy.
** To renew your lost or stolen passport, please make an appointment using our online appointment system.
The requirements to replace a lost or stolen passport are generally the same as if renewing a passport :
** Applicant (or child and both parents) must appear in person
** Original police report from the country where the passport was lost or stolen (with English translation if applicable)
** Proof of U.S. citizenship and/or identity (e.g., passport copy, valid U.S. driver’s license, government-issued photo ID, etc.)
** If under age 16: original birth certificate and both parents’ valid passports
** DS-11 passport form (blank PDF (58 KB) or fill out online and print)
** DS-64 Statement Regarding a Lost or Stolen Passport (blank PDF (58 KB) to print, fill out, and bring)
** 1 passport photo
