Report of Birth :

For infants born in Singapore of Filipino parent(s) :
** Please seek an appointment by sending a message to epassport.sgpe AT gmail.com containing the request for an appointment and the baby’s full name

** kindly note that the time for encoding is between 1:00 pm to 4:00 pm, Monday to Friday.
** The infant will have to be brought to the Embassy for the photo capturing.
** The necessary forms must be filled up by either of the Filipino parents, or the guardian.

Basic Requirements :
** Duly accomplished passport application form. Click here to download this form.
** Cash payment of S$144.50 (passport S$102.00 and notarization S$42.50)

Original and five (5) photocopies of the following documents :
** Birth Certificate issued by the Singapore Registration of Births
** Report of Birth. Click here (inputtable text fields) to download form. All must have the original signature of the parent.
** Data page of passports of parents
** Marriage contract of parents (if applicable)

If the parents are not legally married, please prepare the following documents:
** Affidavit by the father consenting to the use of his surname
** Cash payment of S$187.00 (passport S$102, and notarization S$42.50 x 2)

Note :
** Two copies of the Report of Birth will be forwarded to the NSO and to the DFA in Manila, one copy will be retained by the parents/legal guardian, and one copy will remain in the Embassy for its archives.
** The parent can apply directly to NSO for the child’s birth certificate after a minimum of six (6) months.

** For applicants requesting for pouch list numbers, please disregard the old email address on previous collection slips that indicate php AT pacific.net.sg as the recipient; pacific.net has been discontinued.
** Instead, please use civilregistry.sgpe AT gmail.com

If the parents are not legally married, these documents may be required if applicable :
** Affidavit of Admission of Paternity (AAP) (click here to download template), accomplished by the child’s father, to be signed before a consular officer at the Embassy.

** If the father is not in Singapore, this document must be authenticated either in the Philippine embassy or consulate in the country where he resides or by the Department of Foreign Affairs if he is in the Philippines.

** Affidavit to Use Surname of the Father (AUSF) (click here to download template), accomplished by the child’s mother, to be signed before a consular officer at the Embassy.

** If the father’s whereabouts is unknown, a Personal Handwritten Instrument (PHI, such as a personal letter) written by the father which states his acknowledgement of his paternity of the child and copy of the father’s identification (such as passport).

If the child is being applied for a Philippine passport, the following must be prepared :

Fees (payment must be in cash) : Report of Birth fee SGD 42.50

These additional fees may apply if applicable :
** Legalization of AAP or PHI – SGD 42.50
** Legalization of AUSF – SGD 42.50
** Certificate of Registration fee – SGD 42.50
** Affidavit of Late Registration – SGD 42.50
** Passport fee – SGD 102.00
