Get an Arizona Birth Certificate :
** The Coconino County Public Health Services District (CCPHSD) offers certified copies of Arizona birth certificates at the CCPHSD King Street office in Flagstaff.

** Birth Certificates can be requested in person Monday through Thursday from 8:00 AM to 4:30 PM and Friday from 8:00 AM to 4:00 PM at the

To apply for an Arizona Birth Certificate by mail, the following items must be submitted :
** A completed Application for a Certified Copy of Birth Certificate Available at coconino.az.gov/birthcertificates.
** A copy of the front and back of a valid government-issued picture identification which bears the signature of the applicant or a notarized signature.
Note :
** If the applicant is not the registrant or the parent named on the certificate, proof of eligibility must be included.
** A self-addressed stamped envelope must be included with the request
** Enclose payment of $19.00 per copy.
** Acceptable payment methods are: cashier’s checks, money orders, personal checks, Visa, or MasterCard. NO CASH Accepted.
** Cashier’s checks, personal checks and money orders must be the exact amount and made payable to CCPHSD (Coconino County Public Health Services District)
** There is a $25.00 returned check fee assessed to all bad checks.
** In addition, if the balance remains unpaid after multiple collection attempts, the check may be forwarded to the County Attorney’s Bad Check Program for further collection efforts.
** Credit/debit card payments must include the type of card, full card number, expiration date and 3 digit security code on the application.
** A self-addressed, stamped return business envelope.
** Additional postage may be required if requesting multiple copies
Note :
An incomplete application or any of the following will delay the processing of a birth certificate request:
** No self-addressed and stamped return envelope
** Denied payment method
** Missing or unacceptable identification
Who Can Obtain Birth Records :
** CCPHSD offers birth records only for births that occurred in Arizona.
** CCPHSD can issue these Birth Certificates the same day.
** They are $19 each.
Corrections and Amendments :
** CCPHSD can only correct or amend records for births that occurred from 1997 to present.
** The cost is $27 and includes a Birth Certificate.
** For births that occurred prior to 1997, correction paperwork must be sent to the State Office of Vital Records.
** The cost is $30 and includes a Birth Certificate.
From Another State :
** To obtain a certified copy of a Birth Certificate from another state, you must contact that state’s vital records office.
Vital Records :
** Provide the date and place of birth, which should also include the city, county, and name of hospital where the birth occurred.
** Provide the mother’s maiden last name, and if the mother is married or if the couple is not married but filed a paternity affidavit at the hospital, then include father’s information.
** Provide your relationship to the person on the certificate. (with few exceptions, only immediate family can obtain birth certificates. Please call (928) 679-7272 for more information. The request must be signed by the applicant and you must provide us with a copy of a currently valid, signed government issued picture I.D., such as a U.S. or foreign issued passport, driver’s license, or state I.D.)
** Proof of relationship is required. (e.g. birth certificate, death certificate, trusts, wills, marriage certificates – only originals will be accepted)
