** If you want to donate your organs and tissues for transplant after death, you need to register your decision with the Australian Organ Donor Register.
** The Donor Register is the only national register for organ and tissue donation.

** One donor can save or greatly improve the lives of 10 or more people.
** As a donor, you can choose which organs (kidneys, heart, lungs, liver, and pancreas) and tissues (heart valves, corneas, and bone and skin tissues) to donate.

To register with the Donor Register, you :
** must be aged 16 years or older
** need to submit your personal details, and
** need to submit your Medicare card number.

You can also register your wish not to donate. Donating is voluntary, but family consent is always needed before donation can go ahead, so remember to discuss your decision with family and close friends.

Change or Removal of Organ Donation Decision Details

The option to update your organ or tissue donation decision is no longer available on your driver licence renewal form. If you want to change your details, or change or remove your decision to donate with the Australian Organ Donor Register, you can do this online.

What You’ll Need

** myGov Medicare online account
** your personal details.

How To Change

** Select the ‘Change online’ button.
** Log in to your myGov account.
** Select Medicare from Your services.
** Review your personal details, if all correct, select ‘Proceed to Online Account’ to access your Medicare online account.
** Select ‘Organ donor details’ from the left side navigation menu.

** Choose from the options ‘Change or View my donation decision’ or ‘Remove me from the Donor Register’.
** Complete the online form to request the change or removal.
** Review your donation decision and, if correct, select ‘Submit’ to finalise the request.

If you don’t wish to apply online :
1. Download, complete and sign Change or removal of organ donation decision.
2. Return the form to a Medicare Service Centre, or post your application to :

Organ donation is a life-saving and life-transforming medical process. Organ and tissue donation involves removing organs and tissues from someone who has died (a donor) and transplanting them into someone who, in many cases, is very ill or dying (a recipient).
