Eligibility or Criteria :
** Applicants must attend in person
** Damaged certificate must be submitted to issue a replacement
Required Documents :
** Original civil ID and a copy
** Copy of the nationality certificate, if available
** Letter from the applicant
** Four (4) recent personal photos (4 × 6 cm) with blue background
Procedures :
** If you are a Kuwaiti citizen, visit the General Department of Citizenship and Travel Documents in the governorate you refer to and submit the required documents
** Fill the service application form
** Pay the fees due
Fees :
** Variable fees 0 KD
** Fixed Fees : Replacement for lost certificate 5 KD
** Fixed Fees : Replacement for damaged certificate 1 KD
