Death Registration :
All deaths that occur in the Sultanate and deaths of Omanis that occur abroad are to be reported.

Death that occurs in the Sultanate must be reported to the Registrar within two (2) weeks from the date of death, and within one (1) month if it occurs outside the Sultanate. A death certificate is to be issued to be an approbated official document.
Required Documents for Omanis :
** Death notification from the Health care institute where the death takes place.
** A letter from the sheikh or the chief of the district approved by the wali if the death occurs outside the hospital and no notification could be obtained from the Ministry of Health.
** Death certificate or any attested certificate issued by a competent authority and endorsed by the embassy or the consulate of Oman in the country where death takes place.
** Original ID card or passport of the deceased.
** Applicant’s Identity card or passport.
** Signed notification of the applicant if the identity of the deceased can not be verified.
Required Documents fro Non-Omanis :
** Death notification from the Health care institute where the death takes place.
** Original resident card or passport of the deceased.
** Applicant’s Id card or resident card or passport.
** Signed notification of the informant if the identity of the deceased can not be verified.
** The death notification should be endorsed by ROP hospital.
Informants with legal responsibility to report death are :
** The ascendants, descendants or spouses of the deceased.
** An adult who is the closest relative of the deceased and who was present at the death.
** An adult who shared the same accommodation with the deceased, if the death occurs in that same accommodation.
** The sheikh or chief of the district.
** The doctor who checked the deceased.
** Directors of institutions like hospitals, prisons, quarantines, or other places where death takes place.
** The attorney general when the death sentence is carried out within twenty-four (24) hours from the time of the execution.
** Security and Defense units in charge of informing the General Directorate of Civil Status about their soldiers, civilians and volunteers who die or are martyred in or outside the Sultanate.
Authorized People to receive the Death Certificate :
** The closest relatives to the deceased.
** An adult who is the closest relative of the deceased and who was present at the death.
** An adult who shared the same accommodation with the deceased if the death occurs in that same accommodation.
** The sheikh or chief of the district.
** The doctor who checked the deceased.
Fees :
No fees for new issuance (for first time).
RO (1) for Copy of Original.
RO (5) for damaged/lost.
