It is possible to obtain a copy of the family civil status record by following this procedure:

** Statement on oath of lost or stolen family civil status record
** A copy of the old record or extracts of children birth certificates; where necessary
** If the old record was not submitted by the consulate, it is up to the applicant to provide its place and date of issues in view of applying to the civil status officer of the place of issue in order to obtain a duplicate

Obtaining family civil status book for the first time :
The family civil status book is the only official document which proves the existence of family ties between the different members of a family. It comprises the dates and places of birth of family members.
This document, which is drawn up in Arabic and French, should be requested by husbands upon marriage in order to facilitate reporting birth of future newborn children.

** Original marriage contract
** Full copy of birth certificate with marginal notices about spouses in Arabic and Latin languages
** Statement on oath indicating that the applicant has never obtained a family book
** National identity card of Moroccan wife
** Identity card of foreign wife
** Residence card
** Two photos
For the widow :
** Consular registration card
** Original marriage contract
** Death certificate of husband
** Full copy of birth certificate
** Statement on oath indicating that the applicant has never obtained a family book

For single mother :
