** Two Passport size photos.
** Introductory letter from FBZ customers, employer, Minister of religion/Lawyer, Chief/Head man.
** Introductory Letter from the Public Service Pensions Fund (PSPF), or any eligible Pension Fund Manager
** Proof of residence (e.g. utility bill).
