Apply For Change of Name :
Change of name certificates are only available to:
** the registered person
** the parents/legal custodians/guardians of a child less than 18 years. Documentary evidence must be provided if the child is less than 18 years.

Please note: Access to change of name certificates includes birth certificates with a change of name noted on them.
Historical :
Anyone – Records over 100 years old (from date of birth) are unrestricted. Applications will usually be for family history or similar research purposes.
Family :
Registered Person : No restriction
Parent : Restricted unless child is less than 18 years of age.
Spouse (including Defacto), Child, Brother, Sister, Grandparent, Other Kin : Restricted – No access
Non-parental legal custodian, guardian :
Restricted unless child is less than 18 years of age. Documentary evidence of custody or guardianship will be required.
Estate Matters :
Executor, Administrator, Trustee – Restricted to deceased’s certificate only.
Other :
Anyone – Must have written authority from a person entitled under Family or Estate Matters Access, or alternatively, must be Power of Attorney for a person in one of these categories.
Fees :
** Fees for certificates cover the search of the register – $48.75.
** Occasionally, CBS can’t find a record to match an application. If this happens, a ‘no record’ certificate is issued and a search fee of $48.75 for each ten-year period is charged.
Priority services :
** The extra cost for fast-tracking a certificate is $37. Certificates can be picked up from the CBS Customer Service Centre in Adelaide.
Processing times :
** counter applications – 30 mins
** posted applications – same-day service.

** registered person-to-person post for online applications – $9.15
** international registered post for online applications – $13.45
** express post (within Australia) – $5.70
** international express post – $14.45.

When to apply:
You can apply for a change of name certificate at any time. However, only one change of name is allowed in any 12-month period
