Who can apply:
Anyone born in South Australia can apply for their own birth certificate.

Please note: All births with a change of name noted on them are subject to the change of name access requirements and not the birth access requirements.
Historical :
Anyone – Records over 100 years old are unrestricted. Applications will usually be for family history or similar research purposes.
Family :
Registered Person Spouse Parent Child :
No restriction. Spouse must be married to registered person. In the event of differing surnames, evidence of marriage will be required.
Defacto Spouse Brother, Sister, Grandparent, Other Kin :
Restricted – No access
Non-parental legal custodian, guardian:
No restriction, however documentary evidence of custody or guardianship will be required.
Estate Matters :
Executor, Administrator, Trustee – Applicant must name the testator or intestate deceased and show evidence of executor status and relationship to deceased.
Other :
Anyone – Must have written authority from a person entitled under Family or Estate Matters Access, or alternatively, must be Power of Attorney for a person in one of these categories.
For all but historical access, applicants are required to produce Proof of Identity at the time of application.

The Births, Deaths and Marriages Registration Office has a range of beautiful Commemorative Certificates available for purchase, please see Commemorative Certificates to view the range.
Corrections to certificates :
** Birth, death and marriage certificates are produced from information contained in the Births, Deaths and Marriages Register.
** Consumer and Business Services (CBS) can update information in the register if it’s not accurate. Information such as addresses, ages and occupations are not updated to show recent information.
What you’ll need :
Applicants need one of these to prove their identity :
** current Australian driver’s licence
** social security benefits entitlement card
** current credit card
** defence force or police service photo ID card
** passport
** proof of age card
** current Australian firearms licence.
How to apply :
1. Application form :
** Fill out and sign the application form – Application to correct an entry.
** Do not use this form to change a name or to add the father or co-parent’s details to an existing birth certificate.
2. Supporting evidence :
** Attach a copy of the proof of identity and your supporting evidence to your application.

Correction fees :
** A fee may be charged to fix an error if incorrect information was supplied to CBS. The incorrect certificate will need to be returned.
