When applying for the information you must include:
** payment of $22.00 by cheque, money order or by providing your Visa or MasterCard details – make cheques payable to Registrar of Motor Vehicles
** your name and address
** your licence number
** your date of birth
** the reason why you would like the information.
Your application must be signed and include your fax number if you want the report to be returned by fax.
Courts may request a copy of your driving history. Employers and insurance companies may also request a copy of your driving history, with your written consent.

Check demerit points:
Demerit points are incurred for most motoring offences in South Australia and interstate.
If you accrue 12 or more demerit points in a three year period, you will lose your driver’s licence.
Additionally, if you are on a learner’s permit or provisional licence you will lose your licence if you accrue four or more demerit points.
An offence that attracts demerit points remains on your licence record for three years from the date of the offence.

Replace a driver’s licence :
If your driver’s licence, provisional licence or learner’s permit is damaged, lost or stolen, you need to apply for a replacement.
The replacement is valid for the period remaining on your original licence. For example, if you had a 10-year licence that was issued three years ago, your replacement licence will be valid for seven years.
Licence photographs are generally valid for 10 years. If your photograph is nearing its expiry date, you may need to get a new one before your licence can be replaced.
Driver licence report certified :
** A certified report of your driving history is suitable for legal proceedings – eg court appearances.
How to apply :

** Your application must include how you want the report delivered, by either email, fax or post.
** Please allow up to six weeks for postage to an overseas address.
2. Fee :
The $22 fee can be paid by :
