4. Identity of the deceased (passport or ID card or any official document proving the identity)
5. Identity of the individual who reported the death (passport or ID card)
6. Death notification form
7. Death certificate endorsed by the Kingdom of Bahrain’s embassy or the Ministry of Foreign Affairs or the closest diplomatic representative once in the country in which the death occurred (If the death happens abroad)
