Registry General Apply For Death Certificate

Notice of Death:
The Registry General is notified of a death when it receives a “Notice of Death – Form E” from a medical practitioner or the Coroner. The particulars pertaining to the death are entered into the Death Register.

If a Death Certificate is required one must order it from the Registry General. For new deaths, loved ones can request death certificates from the undertaker who will in turn order the Death Certificates from the Registry General.

For new deaths, a Death Certificate will be processed in 10 working days of receipt of the completed “Notice of Death – Form E” from the undertaker or coroner. For previously registered deaths, a certificate will be processed within 2 working days. Urgent requests for Death Certificates are processed in less than 2 working days for an additional fee.

Members of the public may search the death records at our Public Search Facility on payment of an hourly fee.

Marine Deaths:
The Registrar of Shipping provides details for Marine Deaths which are entered in the Register of Marine Deaths kept by the Registry General.

Notice of Birth:
Adding a Father’s Name to a Birth Entry

Voluntary:
Where a father’s name is absent from a birth entry and the father wishes to have his name added to that child’s birth entry, he and the child’s mother must make a joint request to the Registrar General to have the father’s name added. Proof of paternity may be required.

Putative:
A putative father in a paternity suit is the person alleged to have fathered the child whose parentage is at issue in the suit. A putative father’s name may be added by order of Court.

Adoptions:
An Adoption Order is made in the Magistrates’ Court, in respect of each child who is adopted in Bermuda. The adoption order is sent to the Registry General for the purpose of recording the adoption in the Adoption Register. Thereafter a certificate of adoption may be obtained on application and payment of the required fee.
