Report of Birth :
The birth of a child born to any Filipino parent in Australia must be reported to the Philippine Embassy within 30 days after the date of birth.

If reported after 30 days, the registration is deemed delayed and additional documentary requirements must be submitted.
If the report is sent to the Embassy through mail, have two witnesses write their names, address and signature in the pertinent portion in the Report of Birth:
** Duly-accomplished Report of Birth application form.
** Certified true copies of the child’s birth certificate, issued by Australia’s Registry of Births, Deaths and Marriages.
** Certified true copy of the marriage certificate of the parents (if married), and data pages of the parents’ passport.
** Money order/Bank Cheque of AUD45 (made payable to the Philippine Embassy. If paying by credit card, please provide the credit card name, number and expiry date).
** Pre-paid, self-addressed envelope.
** Please provide your contact details and email address in case the Embassy needs to communicate with you regarding your application.
If reporting 30 days after the child’s birth date, an Affidavit or a Statutory Declaration for the Delayed Registration of Birth must be submitted in addition to the above-mentioned requirements.
The Affidavit/Statutory Declaration must contain the following details:
** Name of Child
** Date and place of birth
** Name of parents
** If parents are married, place and date of marriage
** If parents are not married, father’s name if acknowledge
** Reason for not registering within 30 days after birth.
** Please provide your contact details and email address in case the Embassy needs to communicate with you regarding your application.
If the party reporting the birth of an illegitimate child is not the mother, the following documents must be submitted in addition to the above-mentioned requirements
1. Certified true copies of any two of the following documents which may show the name of the child, date and place of birth, and name of mother (and father’s name if the child has been acknowledged):
** Baptismal certificate
** School records
** Income tax return of parent/s
** Insurance policy
** Medical records
** Other similar documents
2. Affidavit of two disinterested persons who might have witnessed or known the birth of the child
