Please Note:
This a one-way document and will allow you only to enter south africa.
This document can only be collected in person by the applicant.

Instructions:
All applicants for EMERGENCY TRAVEL CERTIFICATES must provide an e – mail address.
** Payments must be made in British Pounds in form of cash, bank draft or postal order.
** Postal orders and bank drafts must be made payable to SA HIGH COMMISSION.
** Application for Emergency Travel certificate (for a child under 15 ½ years of age) made by post must be sent by special delivery envelope and must have another fully paid self addressed special delivery envelope enclosed for the return of issued document.
** Incomplete applications will not be considered.
** Applicants older than 15 ½ must collect this document in person at the office as a thumb print needs to be taken.
** Fingerprints of applicants will be taken by Consular Officers.
** Proof of flight itinerary.
** Address in South Africa.
** A letter and documentarty proof (medical or death certificate) to motivate the application for an Emergency travel certificate.
** Proof of what happened to your SA passport, if lost/stolen a police report. If damaged a sworn statement detailing how your passport was damaged/destroyed.
** Emergency travel certificates will be issued in the last know name and surname as currently reflected on the South African National Population Register.
Opening Hours Are:
The office dealing with South African passport holders will be opened from 08h45 to 12h45 Monday to Friday except on public holidays. Collections will be from 08h45 – 12h45
Required Application Forms:
1. dha-73 (Passport Application Form)
2. bi-529 (Determination of Citizenship)- Must be completed by all SA citizens living outside SA. Holders of foreign citizenship MUST provide proof of how they acquired such a foreign citizenship. In cases of those applicants who acquired foreign citizenship through any of their parents, a certified copy of full birth certificate showing the birth places of your parent(s). Under circumstances where even the parents acquired such a citizenship through descent, a letter from relevant authorities in the country concerned will suffice. Also refer to item 9,10 and 11
3. DHA-9 (Fingerprint Form). The form is completed ONLY by applicants who are 15 ½ years and above. It cannot be downloaded from our website and applicants must sent a fully paid self addressed envelope (Size A4) if they need it. Those applying for passports in person will receive them in the office.
Supporting Documents:
1. Flight itinerary.
2. Physical address in South Africa.
3. 4X passport sized photographs. Please check the photograph specifications before taking the photographs. Applicants who are 15½ years old and those who do not have identity numbers MUST provide 6 passport sized photographs. (In cases where the applicant is not present at the time of application please endorse one photograph)
4. 6X passport size photographs for applicants 15 ½ years of age / who are not in possession of a 13 digit Identity number.
5. 2X certified copies of bio – data page of passport or ID book. This is a page showing your face and all your particulars. Requested copy of the ID book or passports of applicants making applications in person will be certified accordingly by Consular staff. Applicants not having ID number must provide their RSA birth certificate and form BI 24 notice of birth.
6. Submit a Police report relating to the lost/stolen passport. In the absence of a Police Report submit a written explanation as to how/when and where the passport was lost/stolen.
7. Form CMO217 must be fully completed regarding the lost/stolen passport.
8. Written consent from both parents (If applicants are under 18 years)
9. 1X certified copy of marriage certificate – if applicable. Applicants whose documents were issued by South African authorities will be certified by consular officials if they are applying in person. Applicants whose marriage certificates were issued by the UK authorities MUST ensure that their marriage certificates are legalized accordingly.
10. 1X certified copy of marriage certificate – if applicable. 11. 1X certified copy of divorce decree – if applicable. Applicants whose documents were issued by South African authorities will be certified by consular officials if they are applying in person. Those applicants whose divorce decrees certificates were issued by the UK courts MUST ensure that their divorce decrees are legalized accordingly.
12. 2X certified copies of foreign birth registration or naturalization certificate – if applicable.
Additional Requirements For Children Under The Age Of 18:
1. 2X certified copies of applicants full unabridged birth certificates reflecting particulars of both parents. Applicants who will be applying in person at the High Commission will be assisted with the certification of their documents during the application process. They must ONLY have the original document and copy of the document to be certified
2. 2X certified copies of passport and ID of both parents Applicants who will be applying in person at the High Commission will be assisted with the certification of their documents during the application process. They must ONLY have the original document and copy of the document to be certified
3. In case of divorced parents, consent of both parents are required unless SOLE CUSTODY has been awarded to one parent in which case the court order to this effect must be submitted. All documents issued outside of South Africa must be officially translated and legalized by the relevant authorities.
4. Should both parents not be present at the day of submission of the application written consent from the absent parent must be submitted in addition to the signatures on form DHA-73 section C.
