Patient’s Responsibility:
When you come to JFLH for medical attention, you will be required to consult with our Finance & Insurance division. This is required to register your for ancillary services, admission or pre-registration.

Therefore, to ensure that this process is a smooth one, we kindly ask that you bring the following documents with you:
** Your Insurance Cards
** Picture ID
** Social Security Card
** Most recent utility bill for proof of address
** Any related medical information

Please be familiar with your Health Insurance Policy and the coverage it provides. Check with your health insurance carrier about pre-certification requirements to ensure that your procedure/admission will be covered.

Failure to meet these requirements may result in a claim denial and higher co-payment. If you have any questions or concerns, please contact your insurance carrier, or patient representative at the hospital (340) 778-6311 Ext. 2670, 2807 or 2824 for assistance.

Please note that in those instances where a child is injured at school, the school nurse must fill out all required documents. The school insurance forms must be signed by the parents or guardian and the insurance forms must be delivered to the Business Office.

If your insurance is not listed, inquire with our Finance & Insurance staff.

Payment is expected in full if you have no insurance or a co-payment and/or deductible if your insurance coverage is less than 100%.

Self Pay:
If you do not have insurance, payment in full is required within 15 days from receipt of the bill. If you cannot meet this schedule, a Financial Service Representative will be happy to arrange a convenient payment plan.

For patients who reside outside the US Virgin Islands, and do not participate in a medical insurance program accepted by JFLH, a pre-admission deposit is required for elective services, and unless alternative arrangements have been made, all available charges must be paid prior to the patients discharge.

Patients Rights :
All patients admitted to JFL will receive an information package which includes a written copy of the hospital’s statement of Patient’s Rights and Responsibilities and Insurance Notice. The package also includes information on our process of addressing your complaints and/or concerns. This information is also available to you throughout your stay. Additionally, copies of the Patient’s Rights and Responsibilities documents are posted in public areas accessible to patients and their visitors.

Do you know your rights as a patient? As a patient it is important for you to understand that you have rights regarding the privacy and confidentiality of your health information. HIPAA, better known as the Heath Insurance Portability and Accountability Act, is a law that mandates how your health information is accessed, used and disclosed as well as what rights you as the patient are entitled to. Included in the HIPAA law is information about: Hospital Privacy Notice. Disclosures of health information to agencies outside of the hospital. Patients privacy rights. Use of your health information.

Who has access to your health information. If you have more questions about your rights as a patient, would like more information or have a confidential privacy complaint, please feel free to contact the Office of Corporate Compliance at (340)778-6311, ext. 2728. Privacy, it’s your RIGHT! Please note that at JFL we keep your information confidential.

We will ask about your medical history, personal life and treatment as part of your routine care. This information is needed to care for you properly, and like the staff working at JFL, they must follow a strict code to keep your details confidential.
