Death Certificate Online Application :
The following options are available to obtain a certified copy of a Death Certificate

1. Online Application: .
Complete the form, then electronically submit it to the Registrar General’s Office and print the Application Confirmation. After you have been informed that the search was successful, bring the Application Confirmation to the Registrar General’s Office to collect the certified copy.
Please note that you will be required to show identification at that time.
2. By Mail:
Complete the form, then electronically submit it to the Registrar General’s Office and print the Application Confirmation. After you have been informed that the search was successful, mail a copy of the Application Confirmation along with the prescribed payment to:

The certificate will be mailed to you.
3. Paper Application Form:
If you need to give a form to someone who does not have a computer, click here to display the application form. Print the form on your printer, then have the the form filled in by hand and brought to the Registrar General’s Office .
The fees are as follows:
For every certified copy $20.00
For registration of death $10.00
For every general search not directed to any particular entry:
(i) Search by applicant $25.00
(ii) Search by staff of Civil Registry $50.00
For every search for a particular entry $25.00
For correction of each error or omission $25.00
Check-List For Relatives After The Death Of Loved One:
1. After you have been notified by the Hospital of the death, either allow the body to be stored in the Morgue at the Hospital until you have made burial arrangements or
2. Contact the Funeral Home to arrange for the removal of the body from the hospital to the Funeral Home. (At this time, you may inform the Home of your plans for cremation or burial or removal of the body from the Territory.)
3. Obtain from the attending doctor the Medical Certificate showing the cause, time and date of death. Ensure that the causes are legible.
4. Have the death registered at the Registry of Births and Deaths. Note-If the body is to be cremated or removed from the Territory proceed to No. 12 5.
5.Obtain a “Permit to Bury” from the Registry of Births and Deaths
6.If needed, apply for death certificate at the Registry of Births and Deaths.
7. Proceed to the Ministry of Health and Social Development to apply for Permission to use a Burial Ground and to construct a vault if a vault is desired. Take the “Permission to Bury” Certificate to the Ministry as proof of Registration and permission to bury.
8. Contact your Church Minister, Pastor etc. to make arrangements for the Funeral. Take the “Permission to Bury” Certificate to the Minister, Pastor, etc. who will keep it as his authorization to proceed with the burial.
9. Make arrangements with the Funeral Home for burial.
10. Publicize death and funeral on Radio if desired.
11. Make arrangements for the vault or grave.
For Removal Of Body From The Territory Only:
12. Proceed to the Ministry of Health and Social Development and make application for permission to remove the body from the Territory for cremation.
13. Inform the Funeral Home of plans to cremate.
14. Once approved to remove the body from the Territory is obtained, take permission to Registry of Births and Deaths and apply for a Removal Certificate to Remove Dead Body from the Territory.
Note:
The Funeral Home will assist you in making the formal arrangements for= removal of the body whether for burial or cremation. It is therefore advisable to keep the Funeral Home informed of your wishes from the beginning.
