New Registration and Re-registration of Employees :

Basic Eligibility and Requirements :
Members of the public can apply for this in the following event;
Establishment of a new institution

No charge for application forms.

Re- registration :
1 When address of existing institutions are changed.
2 When the name of an existing institution is charged.
3 When the proprietor is charged.

Submission of Application :
Form D should be submitted duly perfected in duplicate. It should be submitted to the Labour Office of the area in which the institution is situated.

Methods of Submission :
All applications for registration of institutions should be addressed to Assistant Commissioner of Labour, Registration Division, Employees Provident Fund, Department of Labour, Colombo 5.

Note: If it is not possible to send all applications for registration of institutions, to the Assistant Commissioner of Labour of the Employees Provident Fund Division, they should be handed over to the District Office.

Application for registration of business

Step by Step Procedure :

Step 01 : Respective applicant to obtain form D for registration from the Employees Provident Fund Division or from the District Labour Office.
Step 02 : Handing over of the perfected application form together with the certificate of registration of business to the Assistant Commissioner of Labour (E.P.F)
Step 03 : Checking the correctness of information furnished by the Employees Provident Fund Division.
Step 04 : Issue of the certificate of registration to the applicant within 01 day.
Note: If incomplete and incorrect information has been furnished, applications will be rejected.

Period to be Taken for the Service :
Period to be Taken for Preparation: A period of 01 day will be required for registration.

Handing Over of Applications :
On week days from Monday to Friday
Hours : From 09.00 a.m to 4.30 p.m.
Holidays : Public holidays.

Period of Validity: From the time of registration until the institution lasts

Expenditure on the Service

Charges: There will be no charge.

Recoveries: Contributions to the Employees Provident Fund should be made in respect of all employees of the institution from the date of their recruitment.

Imposition of Penalties when Payment is Delayed :
Owners of institutions should pay contributions to the Employees Provident Fund in respect of the employees working in their institutions.
Failure to make such payments or defaulting payments will make them liable to pay fines as follows.
** From 1 day to 10 days 5%
** From 10 days to 1 month 15%
** From 1 month to 3 months 20%
** From 3 months to 6 months 30%
** From 6 months to 1 years 40%
** More than one year 50%

Supporting Documents: The certificate of registration of business should be forwarded with the application for registration.
