Conditions
The person must:
** live with a disability that results in a loss of independence or could jeopardize the person’s health and safety in the course of travel that does not require the use of a means of transportation;
** submit, if required, an assessment attesting to a disability that has lasted at least six months. The assessment must be done by one of the following professionals:
** special educator in the health and social services network
** occupational therapist
** nurse
** physician
** optometrist
** physiotherapist
** psychologist

Note:
The Société may ask the holder of a disabled parking permit to provide a new assessment of his or her disability at any time, in order to determine if the person still meets the conditions for obtaining the permit.
To apply for a disabled parking permit:
** complete the Disabled Parking Permit Application form;
** submit the form and the required payment to the Société de l’assurance automobile du Québec by mail.
Note:
Before applying for a disabled parking permit, it is useful to know that the Highway Safety Code allows drivers to stop their vehicles in places where it is normally prohibited in order to allow a person with a disability to get in or out of the vehicle, provided this can be done safely.
A holder must return the permit and the certificate to the Société de l’assurance automobile du Québec when they are no longer required or the holder no longer meets the conditions for obtaining a permit.
Fees
The fees for a disabled parking permit and the accompanying certificate are $16.40. This amount is indexed each year.
Payment may be made by a cheque or money order made out to the Société de l’assurance automobile du Québec.
The fees that may be charged by a health care professional for completing the «Professional Assessment» section of the form Disabled Parking Permit Application are not reimbursed by the Société.
Processing Time:
Allow 4 to 6 weeks for the Société de l’assurance automobile du Québec to process the application and notify the applicant of its decision.
Deadline
For a disabled person or a public institution, the disabled parking permit is valid for 5 years. The applicant receives a sticker that must be affixed to the permit to indicate the year and month of expiry.
However, the holder must return the permit and the certificate to the Société de l’assurance automobile du Québec when they are no longer required or the holder no longer meets the conditions for obtaining a permit.
