Gibraltar Passport Application :

Use this form ONLY if you are residing, working or otherwise have a connection with Gibraltar and wish to apply for a British passport issued in Gibraltar. You will need to bring evidence of your current residence or employment or connection with Gibraltar.

Passport applications can be rejected or delayed because of simple mistakes. Please read the guidance notes enclosed with this form to help avoid delays.

Section 1: What type of passport are you applying for** (put a cross “X” in the correct box)

Section 2: Who is the passport for** Personal Details (to be filled in by all applicants)

Section 3: Previous Passport (fill in this part if you have had a previous passport). If your previous passport was lost or stolen you must also complete the form LSO1 which is available from our office or you can download from gibraltar.gov.gi/passports-and-visas .

Section 4: Parents Details (fill in this part if you are applying for a passport for the first time or if the passport is for a person born on or after 1st January 1983 If both parents named above were born after 31 December 1982 OR were born outside Gibraltar or the UK, we will also need the full name, place, country and date of birth of your grandparents (or details of your parent’s claim to British nationality). Write these details in section 6, or on a separate sheet of paper.

Section 5: Certificate of Naturalisation or Registration as a British Citizen (you only need to fill in this part if you previously had another nationality and you have changed your nationality to British Overseas Territories Citizen or British Citizen)

Caution :
It is a criminal offence to make a false statement to get a passport. If you have made a false statement on this form, you could be prosecuted and could go to prison. Our work includes checking that the countersignature in section 9 is genuine.

I declare that I am 18 years or over (or will be within 2 weeks) and that:
1. I will return the lost passport to the CSRO passport office in Gibraltar or a UK passport issuing office if it comes into my possession;
2. I, or the person named in section 2 of this application (if different) am a British national and have not lost or given up my national status;
3. I do not owe any money to H.M. Government of Gibraltar or to the UK Government for repatriation or any similar relief;
4. This application does not break the terms of any court order to which I, and the person named in section 2 (if different) am subject;
5. As far as I know, all the information I have given in this application is correct;
6. If the application is for a child, I have parental responsibility and I have enclosed any court orders that relate to the child’s residence, contact or removal from Gibraltar/country of residence;
7. I, or the person named in section 2 (if different) understand that by voluntarily applying for a British passport, I may lose my citizenship of another country; and
I have read both the guidance notes and the caution above and fully understand the consequences of my actions in applying for a passport.
I also understand that the Civil Status and Registration Office holds and uses data in accordance with the Data Protection Act 2004. Information about this application may be shared with appropriate public and private sector authorities.

Section 8: Passport Signature & Photo

Section 9: Countersignature (if a countersignature is required, they should fill in this section after the rest of the form has been filled in – see page 10 of the Guidance Notes)

Caution:
It is a criminal offence to make a false statement to help someone get a passport. Check the form properly before you fill in this section. If you have made any false statements on this form, or if you know that the person applying has made any false statements on this form, you could be prosecuted and could go to prison. Our work includes checking that your details are genuine. As a result, we may need to contact you. You should not sign this form if you are a relative of the person applying.

Important :
If the application is for a child, you are confirming the identity of the adult signing in section 7. You must also be able to identify the child in order to certify the photograph.

Endorse the back of one photo – write “I certify that this is a true likeness of ” and then insert the name of the person named in section 2 and then your signature and the date.

What to do next:
Take or send this form to the Nationality and Passport section of the Civil Status and Registration Office, Joshua Hassan House, 3 Secretary’s Lane, Gibraltar, with your current passport the prescribed fee, two photographs and any documents required as shown in the guidance notes at least 10 working days before the passport is actually required. You may be allowed to retain your current passport whilst the application is being processed, but it must be presented for cancellation on collection of the new passport.
Please note that British passports may be issued in Gibraltar only to British Citizens and British Overseas Territories Citizens.

We may check information about you with other information we have. We may get information about you from other people and certain other organisations. We may give information to certain other organisations, as the law allows, to:
** check the accuracy of information;
** prevent or detect crime;
** protect public funds in other ways; and
** use in research or statistics.

These other organisations include other government departments, local authorities, and private sector bodies such as banks and organisations that may lend you money. We will not give information about you to anyone outside our department unless the law allows us to.
