PSH Replacement of Lost ID Card
Identity card of a citizen of Georgia is a document verifying a person’s citizenship of Georgia, identity and legal residence.

ID card of a citizen of Georgia contains two pages, it is printed and filled in Georgian and English languages in accordance with ICAO standards, and in Autonomous Republic of Abkhazia it is filled also in Abkhazian language.
ID card fosters a citizen to receive educational and medical (insurance and other medical services) services, to be integrated in various types of legal relations.
How can I act in case of loss of my ID card?:
You are obliged to replace your ID card in the following circumstances:
** Change of place of residence;
** Expiry of validity for ID card;
** Change of name or surname;
** Determination of inaccuracies in the records;
** Worthlessness for use
** Loss of ID card
While replacement of ID card, you are entitled to:
1. take ID card without change of registration place
2. to be registered on other address based on property document, or householder’s agreement.
Also, evidence for registration is a written confirmation of two majors on residence of a person to be registered within one month period.
In case of loss my ID card, to whom I can appeal?:
In order to receive new ID card, apply to Public Service Hall or relevant territorial offices of Civil Registry
Note:
An applicant may be any person above age 14 or a legal representative of a person under age 14.
In order to take new ID card, you are required to have:
** Birth certificate or identity document;
** Householder’s agreement, property document and written confirmation of two majors on residence of a person to be registered for the last one month (when you have to be registered on other address. Consent of legal representative, if you change registration and you are a minor under age 16).
** Two bio-photos
What is the term of validity for ID card?
Under age 18: 4 –year period
Above the age of 18: 10-year period
Terms and fees to receive ID card:
On 10th working day: 30 Gel
On 5th working day: 40 Gel
On 3rd working day: 50 Gel
In 24 hours: 60 Gel
On the same day: 65 Gel

Who is eligible to pick up the ready Electronic ID Card?
A ready Electronic ID Card shall be delivered to :
** An ID Card holder person of full legal age
** An ID Card holder minor who attained 14 years of age ;
** A legal representative of a minor (under 18 years of age), except for a person from age 16 to age 18 having a registered marriage.
** A guardian of a ward;
Note :
** While submitting an application requesting a document, an interested person may request the document to be delivered by post to the given address.
What kind of preferences might be applied for obtaining Electronic ID Card?
At the time of paying the service fee for Electronic ID Card, the discount or full exemption is applied for :
** Socially vulnerable family members, having the rating point not exceeding 70 001 – full exemption;
** Participants of the second World War – full exemption ;
** Georgian citizens who are disabled veterans due to military operations for territorial integrity of Georgia – full exemption ;
** Foreign citizens, who were granted a citizenship of Georgia on the President’s initiative – full exemption;
** Students, who study at the authorized Higher Educational Facilities and whose status might be checked via the agency database, in case of obtaining Electronic Identity Card with the mentioned status for the first time are exempted from payment of the service fee for the certain service- for 10 GEL, from payment of the photo fee – fully.
** Students obtaining Electronic Identity Card with the student data for the second time – 10%.
