Where can I register the birth of my child?
** Child is younger than 1 year old: Hospitals with an MHAI office, Sub-Regional offices, and some Magistrates Courts
** Child is between 1 and 5 years old (Late Registration of Birth): Sub-Regional offices and Hospitals with an MHAI office
** Child is older than 5 years (Late Registration of Birth): Regional Offices and selected Sub-Regional offices
What can I use if I do not have a “Yellow Health Card” for my child?
** Go to the hospital where your child was born and get proof of your child’s birth
If proof cannot be found, bring as many of the following documents as possible for consideration :
** Baptismal Card
** Sworn affidavit from Headman or School Principal (First school attended by your child)
** Parent(s) acting as a witness (s) and a statement under Oath
Can a parent, who is absent at the time of registration, appear on the Child’s Birth Certificate?
** Yes, only if you bring a Marriage Certificate, or if the unmarried father/mother has provided a sworn affidavit that he/she is the biological father/mother, and
** A certified copy of the absent parent’s identification (ID, Passport or Drivers Licence)
Which parent’s surname can a child take?
** Either of the parents’ surname
** For unmarried parents; the surname of the father will be used only if he and the mother both consent to this. The surname will otherwise be the surname of the mother
If my child’s father is not present at registration, can his particulars be added later?
Yes, it is free to re-register a child with the father’s particulars later
How does an orphaned or abandoned child get a Birth Certificate?
** Provide proof of birth
** Provide copies of the deceased parents’ identifiction and death certificates
** Provide a report from the Ministry of Gender Equality and Child Welfare
