Texas Apply for Nurse Endorsement Application
Applying for an initial license online is convenient and easy, requiring only a few simple steps :

** Enter all requested information.
** Review the information you entered and modify, if necessary.

Guidelines & Eligibility :
** Applicants must have a valid US Social Security Number.
** Applicants must have written and passed one of the required U.S. examinations (See the Eligibility Page for more information).
** Applicants must not have any eligibility issues (See the Eligibility Page for more information).
** Applicants must have practiced in the level of nursing for the applied licensure type, within the last four years or have taken and passed the appropriate U.S. licensure examination for the applied licensure type within four years.
** Applicants must not claim a Compact Primary State of Residence or Compact Home State, other than Texas, as defined by the Nursing Licensure Compact (See the Eligibility Page for more information).
** The application will request information from your Texas Driver License or Texas Identification Card. Please have this card handy, if applicable.
** Licensed Vocational Nurses and/or Registered Nurses, who are not able to use this online application, may download an Application for Licensure by Endorsement from our website (bon.texas.gov/).

** A response is required for each question within this application.
** In order to avoid delays in the application process, please read each question carefully and provide accurate information.
** On this page and throughout the application, the term “applied licensure type” or “Licensure Type” is used to reference the level of nursing, either Licensed Vocational Nurse or Registered Nurse, for which you are submitting this application.
Licensed Vocational/Practical Nurses – Requirements for Texas Licensure :
** Required fee is $186.00. The fee is non-refundable.
** Graduation from high school or hold G.E.D. certificate.
** Completion of an approved school of vocational/practical nursing with a minimum grade of “C” in all nursing courses.
** If you were educated internationally, then our office will need to receive a Credential Evaluation Service (CES) Full Education course-by-course report from the Commission on Graduates of Foreign Nursing Schools (CGFNS), the Educational Records Evaluation Service, Inc. (ERES) or the International Education Research Foundation, Inc. (IERF) validating the type of nursing program you completed.
Note:
** The CES must be dated within one year of issuance by the certification organization and is required for LVN endorsement only. If you were educated as a Registered Nurse, then you are not eligible to apply for an LVN license unless you meet the below exception.
