** The Division of Vital Records issues only certified copies of fetal death records with the raised seal.

Important Information Regarding Right-To-Know Law :
** Fetal death certificates maintained by the Division of Vital Records are not public certificates and, therefore, cannot be released under the provisions of Pennsylvania’s Right-to-Know-Law.

Who May Request a Fetal Death Certificate?
** Only certain people can request a fetal death certificate.
You must be:
** Immediate family member
** Extended family member who indicates a direct relationship to the stillborn fetus

Cost :
** There is a $9 fee for each certified copy.
** This fee may be waived for the dependent of an individual who served or is currently serving in the Armed Forces.

How to Get a Fetal Death Certificate :
** There are three ways to obtain a certified copy of a fetal death certificate.
** Be sure to complete all required information on the application form provided within each link
** Order online
** Request by mail

In person at a public office (see six locations below):
** Erie
** Harrisburg
** New Castle
** Philadelphia
** Pittsburgh
** Scranton

What If The Year Of Death Is Unknown?
** An applicant may request a multi-year death search.

Online ordering is available for most requests; however some exceptions include :
** Birth occured in another country and registered in Pennsylvania.
** Certificate requested for genealogy purposes.
** Fee waiver for Armed Forces member or related to Armed Forces member.
** Certificate of Birth resulting in stillbirth.

** If the reason for your request or your relationship to individual does not appear in the online ordering drop-down box options, then you are not eligible to order the certificate online.
** Please review information on how to apply by mail or in person for a birth certificate or death certificate.

Certificate Of Birth Resulting In Stillbirth :
Who May Request a Certificate of Birth Resulting in Stillbirth?
** A parent may obtain a Certificate of Birth Resulting in Stillbirth by completing an Application for Certified Copy of Death Record.
** Parent is to write “Certificate of Birth Resulting in Stillbirth” in Part two, Intended Use of Certified Copy.

** Since the name of the stillborn fetus was not included on the Certificate of Fetal Death until 2006, the mother or father may provide on the application form the name of the stillborn fetus to be listed on the Certificate of Birth Resulting in Stillbirth, provided that if a name is given it must be the same as the name on file with the Department, if any.

** The first copy is available at no charge. There is a $9 fee for each additional copy requested.
** This fee may be waived for the dependent of an individual who served or is currently serving in the Armed Forces.

How to Get a Certificate of Birth Resulting in Stillbirth :
** There are two ways to obtain a Certificate of Birth Resulting in Stillbirth.

Be sure to complete all required information on the application form provided within each link :
1. Request by Mail (Note parent is to write “Certificate of Birth Resulting in Stillbirth” in Part two, Intended Use of Certified Copy.)

2. Certificate of Birth Resulting in Stillbirth applications may be filed in person at a public office; however, the Certificate of Birth Resulting in Stillbirth will be mailed from the central office in New Castle, Pennsylvania.
