How do I get my certificate for Marriage & Divorce :

** The Tennessee Office of Vital Records reviews, registers, amends, issues and maintains the original certificates of births, deaths, marriages and divorces that occur in Tennessee in accordance with Tennessee Code Annotated.

Note To Parents :
** Certificates of Birth for newborn children may not be available to issue for approximately 45 days after this office receives the original certificate from the hospital or birthing facility.
** Expedite fees do not apply to the registration of the birth record, only the issuance of a certificate that is already registered.
** If parents have travel plans which will require a passport for newborn children, travel plans should be made based on this timeline.

** Request for Certificate¹ – By Mail – 6 weeks
** Request for Certificate¹ – By Mail (expedited) – 2 days
** Acknowledgement of Paternity – 2 months
** Adoption – 4 months
** Birth Amendment – 3 months
** Court Ordered Name Change – 3 months
** Death Amendment – 3 months
** Delayed Registration – 3 months
** Marriage Amendment – 3 months
** Divorce Amendment – 3 months

Identification Requirements :
Identification Requirements : (Photocopies Acceptable)
** If the application is signed and notarized, no additional identification documents are required.
** If the actual mother’s copy of a birth certificate for the current year and previous two years is submitted as the application and the response is being mailed to a parent, no additional identification documents are required.

Accepted Forms of Identification :
One item from the following list :
** Current drivers license, including the issue and expiration date.
** Current passport
** Military I.D. card
** Alien, temporary or permanent resident card
** Employment authorization card
** U.S. Certificate of Naturalization
** Certificate of Citizenship or Citizenship I.D. card

Two items from the following list :
** Current pay stub or W2
** Vehicle registration with name and current address
** Voter Registration card
** Military Discharge (DD214)
** Utility Bill, Bank Statement or deposit slip with name and current address
** Health care coverage card
** Medical record
** Application page of an insurance policy
** Signed Social Security Card
** For those person who have had their I.D. stolen, a copy a police report or other official documents which support the theft.

By Mail :
What to include :
** The completed and signed application for certified copy of certificate of birth, death, marriage or divorce

One of the following for identification purposes :
** a photocopy of a government-issued identification card which includes the signature of the requester
** a notarized application for certified copy
** the mother’s copy of the birth certificate (issued at the hospital at the time of birth) may be used for identification when obtaining a newborn’s birth certificate.(It must be dated within the last two calendar years.)
** if none of these documents are available, see ID requirements for alternatives
** A check or money order made payable to Tennessee Vital Records for the appropriate fee.
