Order a Birth Certificate :

Registering a Birth :
** You need to register your baby’s birth before you can get a birth certificate.
** When your baby is born, your nurse will help you fill out a form with your baby’s information including his or her name and parents’ names.

** After you register your child’s birth, you can apply for a birth certificate.
** You can get help filling out forms at your local health centre.

Birth Certificate :
A birth certificate is an important personal document that may be used for :
** Legal identity
** Age, sex or nationality
** Travelling to other countries or applying for a passport
** Applying for a Nunavut Health Card, a Social Insurance Number, or a driver’s license
** School enrolment
** Settling an estate
** Accessing old age pension benefits

You can get a birth certificate if you are :
** The person named on the birth registration
** Named as a parent on the live birth registration
** A legal representative (e.g. guardian, solicitor, or legal agent) of the person whose name is on the birth registration
** The spouse of the person whose name is on the birth registration
** An agent, or any other person, with written authorization of the person concerned or the parents
** A person who requires the certificate for use in a court of law, for adoption, or for settlement of an estate
** An officer of the Crown for official purposes
** A government department approved by the Registrar General of Vital Statistics, when required for legal or other legitimate purposes

Get a Birth Certificate :
** The Department of Health can provide birth certificates dating back to the creation of Nunavut on April 1st, 1999.
** To get a birth certificate, fill out the Application for Certificate form. Make sure that all of your information is correct.
** The fee for all birth certificates is $10.00 per certificate and can be paid by cheque or money order made payable to the Government of Nunavut.

Note :
** If you are applying for a passport you will need the large birth certificate.

Medical Travel :
The Department of Health offers medical travel benefits to make sure you have access to medical services that are not available in your home community.

If a doctor, nurse, dentist or other health care professional tells you that you have to leave your home community for medical treatment, you may be entitled to medical travel benefits.
Medical travel benefits help pay for medical services that are offered in communities other than your own.

Eligibility :
** Nunavummiut not eligible for assistance under any other source of funding can apply for medical travel assistance with the Government of Nunavut.

In order to be eligible for travel assistance from the Government of Nunavut, you must be enrolled in the Nunavut Health Care Plan and meet the following conditions :
** A health care professional in your home community must approve you for medical treatment outside your home community. This must be done before you travel

** The medical treatment must be urgent and must be needed before you are travelling for other reasons
** You must have used up all other third-party and employer insurance options or have no insurance plan

Paying for Medical Travel :
Three programs offer benefits to help you pay for the cost of medical travel :
** Nunavut Health Care Plan (NHCP) offers limited medical travel benefits to eligible residents.
** Non Insured Health Benefits (NIHB) provides medical travel to Nunavut Land Claim Beneficiaries.

** Extended Health Benefits (EHB) offers medical travel benefits to help cover costs that are not covered by the Nunavut Health Care Plan and helps if you have used up other insurance benefits like NIHB or employee benefits.

** Medical travel costs may also be paid by the Workers’ Safety and Compensation Commission (WSCC), your employer health benefits or other insurance programs.
