Correct a Birth Certificate :
** Corrections to birth records shall only be completed when required evidentiary documents are submitted or when an administrative error is made.

** Most counties provide vital records services and can correct birth records for birth years of 1997 to the present year.
** Please reference the local county health departments regarding information on their hours of operations, address, and acceptable methods of payment.
** Documents submitted to support a correction to a birth record must be translated in English by a certified translator.
** The translator must sign the translation in the presence of a notary and it must contain the notarial language.
** The state or county Vital Records shall make corrections to birth records only when required evidentiary documents are submitted or when an administrative error is made.
** Most counties provide Vital Records services and can correct birth records with birth years of 1997 to the present year.
** Please reference the county office listing regarding information on their hours of operations, address, acceptable methods of payment, etc.
** For births that occurred in a hospital, parents may contact the hospital directly to request a review of the hospital worksheet.
** A birth record may be corrected if the birth occurred at a hospital and it is determined to be the hospital’s administrative error.
The following is required to correct a birth certificate if not an administrative error :
Birth to Less Than One Year :
To correct the registrant’s information the following is required :
** The Affidavit to Correct or Amend a Birth Certificate must be completed and the form must be signed in the presence of a notary and state the information that currently appears on the birth record and the correction requested.
** A completed and signed Arizona Vital Records Request for Copy of Birth Certificate.
** A copy of the front and back of your valid government issued picture identification which bears your signature or have your signature notarized on the Arizona Vital Records Request for a Copy of Birth Certificate.
** The correction fee. Please reference the fee schedule for the appropriate fee.
** Acceptable payment methods are cashier’s checks, money orders, Visa, or Mastercard.
** Please Do Not Send Cash. Cashier’s checks and money orders must be for the exact amount and made payable to Office of Vital Records.
** If you pay by credit/debit card, you must include the full card number and expiration date on your application.
** Pre-paid postage envelope.
One Year or More After Birth :
To correct the registrant’s information the following is required :
** The Affidavit to Correct or Amend a Birth Certificate must be completed and the form must be signed in the presence of a notary and state the information that currently appears on the birth record and the correction requested.
** At least one independent factual (evidentiary) document that supports the changes requested on the affidavit.
Some examples of acceptable evidentiary documents may include, but are not limited to :
** Hospital or doctor’s office records
** School registration record
** WIC record
** Tribal Census Record
** Health Insurance record
** Baptismal record
** Military record
** SSA Record (not a Social Security Card)
** Immunization records that are doctor’s office print-outs (handwritten-in-book versions are not acceptable).
** FAA (welfare) record
** Certified Court Order
** A completed and signed Arizona Vital Records Request for Copy of Birth Certificate.
** A copy of the front and back of your valid government issued picture identification which bears your signature or have your signature notarized on the Arizona Vital Records Request for a Copy of Birth Certificate.

Parents’ Information :
** The parents’ information may be corrected anytime.
** To correct typographical errors such as transposed letters in the parent’s name or transposed numbers in the date of birth,
some examples of acceptable evidentiary documents may include but are not limited to :
** Parent’s certified birth certificate
** United States or Mexican Passport
** Certified Court order
To correct the parents’ information the following is required :
** The Affidavit to Correct or Amend a Birth Certificate must be completed and the form must be signed in the presence of a notary and state the information that currently appears on the birth record and the correction requested.
