Apply for Death Certificate :

** The Oklahoma State Department of Health Division of Vital Records is responsible for registering every death which occurs in the State of Oklahoma;

** as well as preserving, amending and issuing certified copies of those records in accordance with state law.

What Are Your Business Hours?
** We are open Monday through Friday from 8:30am – 4:00pm.
** We are closed on State Holidays.

Who May Apply For A Death Certificate?
** By state law, death records filed are not open for public inspection.
** Obtaining a death certificate by or for fraudulent purposes is punishable by law.

Death Certificates
  Search of files and the issuance of one (1) certified copy if certificate is on file $15.00
  Additional copies (each) $15.00
Amendments to Death Certificates
   First Certified Copy $35.00
   Additional copies (each) $15.00

 

The person requesting the certificate must meet one of the following eligibility standards :
Person Acting the Subject’s Best Interest
** A surviving spouse, parent, child, grandparent, sibling, or legal guardian;
** Legal representative of the estate of the deceased as documented by an order from a court of competent jurisdiction;
** An individual who can establish a familial relationship with the deceased;
** Funeral director of record or agent thereto, working in the capacity of their official business;
** Person with a court order from a court of competent jurisdiction;
** A person who was a co-owner or a joint tenant on real or personal property of the decedent;
** A person listed in a will of the decedent, provided the will is in probate; or

What Identification Is Required?
** A photocopy of a valid government issued photo ID by either the applicant or an individual attesting for the applicant is required for issuance of certificates.
** If you are mailing your application – Send a photocopy.
** Do not send your original ID.
** Photocopies must be legible.

** U.S. Issued Driver’s license or Identification card (current or expired less than 3 years ago)
** U.S. Passport
** Foreign Issued Passport with Visa (I-94)
** Government issued Military photo ID
** Tribal Photo Identification Card containing the bearer’s signature
** Oklahoma Self-Defense Act (SDA) License or OK Concealed Carry permit
** Resident Alien Card (Form I-551)
** Employment Authorization Card (Form I-766)
** Employment Authorization Card (Form I-688A)
** Temporary Resident Card (Form I-688)
** Oklahoma Dept of Corrections Consolidated Record Card (CRC)

How Do I Apply For A Death Certificate In Person?
** Download an application for a copy of a death certificate (Application)
** Complete the application in full. (any blanks will cause delays in completing your request)
** Bring your photo identification. (Acceptable Forms of Identification)
** Bring correct fees. (Fees)

How Do I Apply For A Death Certificate By Mail?
** Download an application for a copy of a death certificate (Application)
** Complete the application in full. (any blanks will cause delays in completing your request)
** Enclose a photocopy of your photo ID. (Acceptable Forms of Identification) Do not send your original ID.
** Photocopies must be legible.
** Enclose correct fees. (Fees)

How Do I Apply For A Death Certificate Online?
** Oklahoma Department of Health does not accept online orders directly; however, for your convenience, you can process on-line requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc.

** VitalChek can be reached either through its website or by Phone at 877-817-7364.
** An additional fee is charged by VitalChek for using this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.

How Much Does A Certified Copy Of A Death Certificate Cost?
** The fee for the search of a death certificate is $15.00 and will include the issuance of one certified copy to a qualified applicant if the certificate is on file.
** Additional copies will be issued at a cost of $15.00 each.
** The searching fee is not refundable or transferable. All fees expire one (1) year from the receipt date.
** Please make your check or money order payable to OSDH.
** Corrections or additions to death certificates will require additional fees and the submission of required documentation.
** Instructions and assessment of fees will be made on an individual basis after your application has been received and reviewed.

How Long Does It Take To Get A Certified Copy Of A Death Certificate?
Note :
** Below are the average processing times for general issuance, however requests which require certificates to be amended or replaced (e.g. adoption) will take longer.
Mail: allow at least 4 weeks
In person :
** Allow at least 1 hour (some processes will require additional time).
** For quickest service, please arrive early in the morning.
** Expedited Service Requests using the U.S. Postal Service’s Priority Mail Express, will allow processing of most requests within 1-3 business days.

Note :
** a request for an amendment or correction will take an additional 7-10 days.]
** Return of a certificate by the U.S. Postal Service’s Priority Mail Express Requires that an envelope is attached with proper postage.
** Courier services such as Federal Express, UPS etc., will be accepted but items will not be returned through these services.
** Documents will be returned using regular U.S. Mail.
** Death certificates will not be faxed.
** Priority mail will be handled the same as regular mail.
