If you mail in a request for a vital record, you must include:
** A photocopy of the applicant’s ID (or alternative documents)
** The applicant’s reason for needing the record
** The signature of the applicant for the record
** We require these items to screen more thoroughly for fraud.

** Knowing the reason for ordering assists us in reviewing requests for a large number of records, determining whether the applicant has a right to order death, marriage, or divorce records, and whether to send a short or long form certificate.

** Having applicant signatures ensures that records are being requested by eligible persons.

** Please notice that the second page of each form provides ordering instructions, as well as suggestions for alternative ID when the applicant does not have a current valid driver’s license or passport.

Acceptable Proofs of Identity :
Any ONE of the following documents:
** Current, valid Oregon driver’s licence, permit, or ID card.
** Current, valid passport.
** Current, valid out-of-state driver’s licence or ID card.
** Current student body card with name and picture (high school students only).

OR…
Any ONE of the following documents, plus ONE MORE document showing your current address:
** Armed Services ID card.
** Selective service card or military discharge paper (must be original).
** U.S. Alien registration card with picture.
** U.S. Immigration or naturalization papers.
** Tribal membership or ID card with picture (without picture accepted as one of three pieces of ID below).
** Concealed weapon permit with picture.
** Matrícula Consular card.
OR…
Any THREE of the following documents, as long as one shows your current address:
** Official papers issued by courts of record with include date of birth.
** Official corrections department or parole papers showing date of birth and identity.
** Vehicle registration or title.
** Personalized check, savings account passbook, or monthly account statement.
** Check guarantee card. (NOT a debit card. Must say “Check Guarantee Card”.)
** Pistol or firearms permit.
** State hunting or fishing license.
** Recent utility or other bill with current address.
** Recent paycheck stub or paycheck. (Official company type only.)
** Company identification card.
** Voter registration card issued by a county elections department.
** Valid food stamp, welfare, or unemployment identification.
** Medical or hospital card of identification.
** Tax statements – W-2 forms, etc.
** Auto insurance policy.
** Union membership card.
