MDHHS Heirloom Birth Certificates
** You can celebrate a newborn or memorialize an ancestor with an Heirloom Birth Certificate issued by the State of Michigan.

** These certificates are signed by the Governor, printed over artwork and are suitable for framing and preserving as family heirlooms.
** Heirloom birth certificates are not intended as legal proof of birth.
** The certificates are mailed encased in cardboard shields to ensure protection.
Who Can Order :
Heirloom Birth Certificates may be issued only to the following individuals :
** The child named on the record (if 15 years of age or older)
** A parent named on the record.
** Legal guardian
** An heir of a deceased individual.
** Birth records that are more than 100 years old are available to anyone who applies
** You can also purchase a gift certificate for an heirloom birth certificate that can be given as a gift.
** The recipient of the gift certificate will need to complete an application and select the heirloom design of their choice.
How to Order :
** Complete the Heirloom Birth Certificate Application providing all information requested.
** The same application can be used to order a gift certificate.
** Provide a copy of your valid photo identification.
** Birth records less than 100 years old are restricted documents and you must prove eligibility in order to request one.
** We will accept a current valid driver’s license, state-issued photo identification card, or passport.
** Include a check or money order for $40.00 made payable to the “State of Michigan”.
** The fee covers the search for the record, preparation of one heirloom certificate and mailing the certificate via regular mail.
** This fee also includes a $20 contribution to the Children’s Trust Fund.
** Processing time for orders is five (5) weeks.
** This includes mail time and processing your payment through our Accounting Department.
** Most questions about ordering are answered in the Heirloom Birth Certificate FAQ document or the Heirloom Birth Certificate Brochure.
** However, if you have further questions, please contact our Eligibility Unit at 517-335-8666.
Applying In Person :
** Heirloom birth records cannot be processed same-day.
** The application should be mailed to the address listed be-low.
** Or, if you wish, you may come in to our Lansing office and drop off an application, and it will be processed and mailed to you within 4-5 weeks.
Processing Time : Heirloom Birth Records
** The processing time for an heirloom birth record request will be approximately 4-5 weeks, depending on the volume of requests received.
** Same day service is not available.
Payment Information :
Search Fees Are Non-Refundable :
** Fees are established by state statute.
** A basic one year search fee includes either one non-certified copy of the heirloom rec-ord or an official statement that the record is not filed with the state.
** A basic statewide search includes the files for the year specified as the birth year.
At least one of the following ID’s is required:
** Current driver’s license with photo if unexpired, or expiredless than one year and issued within the last five years
** Current state issued photo identification card unexpired, orexpired less than one year and issued within the last fiveyears
** Unexpired U.S. or foreign passport
** U.S. military photo identification or military dependent photoidentification with current expiration date
** Employment identification with photo, accompanied by a cur-rent pay stub or W-2 form
** Department of Corrections photo identification card, accompa-nied by probation or discharge papers dated within last year
** If a currently incarcerated prisoner, a Department of Correc-tions photo identification card, accompanied by a verificationof incarceration by the facility on letterhead
** or persons age 15-20, current student photo ID with either areport card or transcript
