Order a Birth Certificates :
** A legal copy of a birth certificate cannot be issued without PDF proper purpose.
** You may obtain a birth certificate for yourself, your spouse, your parent, or your child.

** Be sure that you meet the above proper purpose before applying for a copy of a birth certificate.
** The fee for the search will be kept if no record is found or the right person has not asked for the record.
** You will be notified in writing if no record is found or the proper purpose has not been met.
** Internet applications for a birth certificate (Applications may be charged to a Visa or MasterCard).
** The person making the application must fax an enlarged and readable photocopy of their current government issued photo identification, i.e. current driver’s license, to 402-742-2385 or attach the current government issued photo id to an email and send to DHHS.VitalRecords AT nebraska.gov .
** It takes 7 to 10 working days to process the application.
** The “Certificate of Birth Resulting in Stillbirth” Application must be applied for by mail or walk-in, not through the Internet or VitalChek.
To get a birth certificate by mail, send a $17.00 check or money order made out to “Vital Records” (no credit cards) and send the following information :
** Child’s full name on record.
** If adopted, list adoptive name and state “adopted”.
** Month/day/year of birth
** City or town of birth/County of birth
** Father’s full name, if adopted, list adoptive father’s name.
** Mother’s full maiden name, if adopted, list adoptive mother’s maiden name.
** Why record is needed?
** If not your record, how are you related
** A photocopy of the current government-issued photo id of the person making the application, i.e. current driver’s license
Certificate of Birth Resulting in Stillbirth :
** A legal copy of Certificate of Birth Resulting in Stillbirth cannot be issued without PDFproper purpose.
** Only the parent(s) of a stillborn (fetal death) may obtain a certified Certificate of Birth Resulting in Stillbirth.
** Be sure that you meet the above proper purpose before applying for a copy of this certificate.
** The fee for the search will be kept if no record is found or the right person has not asked for the record.
** You will be notified in writing if no record is found or the proper purpose has not been met.
To get a Certificate of Birth Resulting in Stillbirth by mail, send a $17.00 check or money order made out to Vital Records (no credit cards) along with the following information
** Child’s name on record,
** Month/day/year of stillbirth,
** City or Town of stillbirth/County of stillbirth,
** Father’s full name,
** Mother’s full maiden name,
