Obtain Birth & Death Record :

New Requirement For Certificates And Verifications :
** Effective March 28, 2016, the Office of Vital Records will be requiring proof of relationship to ensure the State Registrar is satisfied that the applicant has a direct and tangible interest in the recorded matter pursuant to Nevada Revised Statute (NRS) 440.650.

** The Office of Vital Records is dedicated to ensuring the security and confidentiality of all birth and death records as required by state law.

** The Office of Vital Records has created the link below to provide the applicant more information regarding who can receive a certified copy of a certificate or a verification of a record and the proof required by our office.

Direct relationship by blood is defined as :
The person of record :
** Proof required – Proper identification.

A parent listed on the certificate to the person of record :
Proof required – Proper identification.

A child to the person of record :
Proof required – The birth certificate of the applicant.

A brother or sister to the person of record :
Proof required – The birth certificate of the applicant showing both the same parents.

A grandparent (if their child is listed on the record) to the person of record :
Proof required – The birth certificate of their child.

A grandchild to the person of record :
Proof required – The birth certificate of the applicant AND the birth certificate of the applicant’s parents.

Direct relationship by marriage is defined as :
Spouse or domestic partner to the person of record :
** Proof required – The marriage license or certificate of domestic partnership.

Step parent to the person of record :
Proof required – The marriage license or certificate of domestic partnership.

Step children to the person of record :
Proof required – The parent’s marriage license or certificate of domestic partnership AND the applicant’s birth certificate.

Legal relationship is defined as :
Attorney or legal counsel to the person of record :
** Proof required – A copy of a court order or document.

Public Administrator or Guardian to the person of record :
** Proof required – A copy of a court order or court documentation.

Legal Guardian to the person of record :
** Proof required – A copy of a court order.

Adoptive parents to the person of record :
** Proof required – Proper identification.

Adoptive child to the person of record :
** Proof required – The applicant’s birth certificate.

Facilitate a legal process is defined as:
For a law enforcement investigation :
** Proof required – A request on department letterhead which includes the department case number.

For any criminal or civil court proceedings :
** Proof required – Court documentation such as a petition showing court proceedings.

For other legal matters such as to settle an estate, survivor benefits or release of lien or debt :
** Proof required – Court documentation, insurance documentation or other proper documentation.

Birth / Death Certificates :
** The Office of Vital Records is the legal custodian of birth and death certificates filed in the State of Nevada from July 1, 1911 to the present.
** For events prior to 1911, records are usually held in the county Recorder’s office where the event occurred.
** A list of the county Recorder’s offices can be found on our application for a marriage or divorce verification record under “forms”.
** Birth and death records are confidential in the State of Nevada and may only be released to a qualified applicant.
** A qualified applicant is generally defined as the registrant, or a direct family member by blood or marriage, his or her guardian, or his or her legal representative.
