** The vital records system is a one-stop service for purchasing certified copies of birth, death, marriage, domestic partnership and termination of domestic partnership records where the event occurred in Milwaukee County.

Processing vital records applications by mail or online:
** The Milwaukee County Register of Deeds Offers two options for processing vital records applications that do not require visiting the courthouse in-person.
** Select the appropriate application form below, print it on your printer and mail it to us with a cashier’s check or money order. (Sorry, no personal checks.)
** For your convenience, you can process online requests through an independent company that Milwaukee County Register of Deeds has partnered with to provide you this service; VitalChek Network, Inc.
** VitalChek can be reached through their website.An additional fee is charged by VitalChek for this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.”
Free birth record available for the purpose of obtaining an identification card for voting purposes :
To apply for a free birth record in-person or via U.S. mail two forms must be submitted –
Form #1 – Request For Free Birth Record
Form #2 – Birth Certificate Application
** Applicants must be at least 17 years of age
** Only one free birth record will be given per applicant.
** Free birth records will be given only to persons who are current residents of Milwaukee County.
** Non-residents born in Milwaukee County are not eligible.
** To qualify to receive a certified copy of a birth certificate, applicants at the counter will be asked to present two items of secondary identification.

Processing vital records applications in-person :
** Certified copies of vital records are sold and available the same day in the Register of Deeds office from 8:00 AM to 4:15 PM, Monday through Friday excluding major holidays.
Costs are as follows :
** $20.00 each for the first copy of a birth, marriage, death, domestic partnership or termination of domestic partnership certificate
** $3.00 each for additional copies of the same record purchased at the same time
** No Personal Checks Are Accepted
** Credit and Debit Cards are accepted until 4:00 PM
** Credit and debit cards are not accepted for genealogical requests
** To receive a certified copy of a vital record in person, you must present proper identification.

Primary Identification requires ONE of the following:
** Wisconsin Driver’s License with photo
** Wisconsin I.D. with Photo
** Out-of-state Driver’s License/I.D. with photo
Secondary Identification requires TWO of the following:
** Government issued employee ID Badge with photo
** Passport
** Check / Bank Book
** Major Credit Card
** Health Insurance Card
** Recent dated, signed Lease
** Utility Bill or traffic ticket
** All primary identification and at least one of the two secondary forms of identification must show your current name and address.
** Expired cards or documents are not acceptable.
** At the discretion of Vital Records staff, you may be asked to make and sign an identifying or corroborating statement.
** A valid street address, not a P.O. Box, is required for all counter applications.
