Obtaining a Birth or Death Certificate :
** Effective January 2015, the Pima County Office of Vital Records is now able to issue both imaged and computer certificates for any birth or death that occurred in Arizona.

** If you wish to obtain the record in person you must prove eligibility (Arizona is a closed record state) and have a valid government issued picture/signature identification.
** You must also be in our office by 3:00 pm for same day service.
The Pima County Office of Vital Records DOES NOT issue certificates for the following :
** Birth & death certificates from different states – please contact the respective state’s vital records office
** Pima County marriage and divorce records – please contact the Pima County Clerk of the Superior Court
** Adoption applications – please contact the Pima County Juvenile Court
In Person :
** Applicant must provide a valid ID (see identification requirements).
** Check proof of eligibility to see if additional documents are required.
** Bring the appropriate fee ($20 each)
** When you come in, take a number in the lobby and fill out an application form available by each customer service window.
** You may wait for a couple of hours during our busiest times such as the back-to-school summer season or lunch time. Please plan accordingly.
** For same day service, you must be in our office before 3:00 p.m.
By Mail:
** Pick up an application form at our office or print a form out from the internet.
** Please provide a valid notary signature and seal on the application form OR a copy of applicant’s valid ID (see identification requirements).
** Check proof of eligibility to see if you need to mail any additional documents.
** Mail the appropriate fee. Please allow 1-2 weeks from the time you mail your application
** If you wish to have expedited delivery, please send us a self addressed stamped envelope (Examples: USPS Overnight, FedEx, Certified Mail)
** Where to write for out of state certificates.
Valid Government Issued Identification with Picture and Signature :
** United States issued Drivers License or Identification Card
** United States Territories Drivers License or Identification Card
** United States Military Identification Card that contains bearer’s signature (some do not)
** Tribal Identification that contains bearer’s signature
** Passports – United States or Foreign issued (passport must be signed)
** Visa – In a passport and the bearer’s signature is on the passport not the Visa
** Permanent Resident Card issued on or after May 11, 2010 which includes a signature
** Employment Authorized Card (Forms I-766, I-688A or I-668B)
Note :
** Expired, cracked, broken, taped or faded identification cannot be accepted.
Eligibility Requirements for Certificates :
** For proof of identity, please see Identification Requirements.
** Please be aware that hospital copies or uncertified birth certificates cannot be accepted.

Grandparent | A copy of your son/daughter’s certified birth certificate naming you as parent. In addition, your son/daughter must be named as parent on your grandchild’s birth certificate. | A copy of your son/daughter’s certified birth certificate naming you as parent. Spouse (18+) | A copy of your certified marriage certificate, notarized letter from spouse authorizing spouse to obtain birth certificate and copy of spouse’s valid government issued picture/signature identification required. | A marriage certificate that links the surviving spouse to the deceased. |
