How to obtain a copy of a Death Certificate :
Certified Copy Center for:
** 1980 to Present Missouri Certificates of Death for All Missouri Counties,including St. Louis City (standard issue legal document generated from Missouri Vital Records Database)

Who may apply for a copy of Death Certificate :
** A member of the deceased immediate family, which includes those family members and in-law in direct line of decent up to, but not including, cousins.
** His/Her guardian or official representative ie: Attorney, Funeral Director, Doctors
** All family members, genealogist representing a family member and professionally recognized genealogist.
Preparation :
** You must have a valid Driver’s License, State Identification Card, or other acceptable government issued photo identification to apply for a Missouri Death Certificate copy in person.
** Your address on Application (copy request form) must match your address on photo identification.
** If you do not have photo identification, a qualified family member with photo identification may apply for the copy.

** 1910 – Present Death Certificate Copy Request
** Pre – 1910 Death Transcript Copy Request
** As of 02.28.2011, the Missouri Department of Health requires notarization of your signature on the Mail-In Application.

** Include completed application form, a legal size, self-addressed stamped envelope and check or money order for the search fee. Do not send cash.
** Please do not call for record look-ups.

Fees :
** Non-Refundable $13.00 for each 5-year search using Decedent information provided by Applicant (Customer) and, if record is found, one (1) Certified Transcript will be issued.
** $10 for each additional copy
** Mail-In Customers Add 50ยข for return mail if you do not provide Self-Addressed-Stamped-Envelope
Payment Method :
** We will accept MasterCard and Visa (No American Express) for amount of Purchase or Deed Recording Fee.
** Minimum $1.50 service fee Walk-in services only.
** No cash back.
** The service fee for all credit cards will be set at 2.95% of the payment amount with a minimum service fee of $1.50 will be charged for all transactions below $52.
** Service fee pricing for electronic checks: The service fee for electronic checks will be set at $1.50.
This pricing includes the ability for Forte payments to :
** Validate the saving and checking account number real time.
** Validate that there are positive funds in the banks account at the time of the payment.
** The product called ATM Verify validates 95% of all US banks.
** The city will received $.050 per transaction, which will be deposited to the city’s’ designated bank account on the monthly basis.
