How to request a certified record by phone :
** Customers can call a toll-free number (866) 809-0290 from 6:00 a.m. CST through 7:00 p.m. CST, Monday through Friday, except for holidays.
** A fee of $20.00 is required for the record search and includes one copy if a record is on file in our office.
** Each additional copy of the record is also $20.00. A VitalChek operator will take your information, screen your credit card, authenticate your identity and complete your order.

** The fee to screen your credit card and authenticate the caller is an additional $13.00.
** If you request a group order which consists of more than one Vital Event (i.e. Birth, Death or Marriage) within one order there is a fee of $3.00.
** Credit cards currently being accepted include American Express, Discover, MasterCard and Visa.
** Your request may take approximately 10 to 20 business days to process.

How to request a certified record online :
** For your convenience, you can process online requests through an independent company that the Iowa Department of Public Health Bureau of Vital Records has partnered with to provide you this service; VitalChek Network, Inc.
** VitalChek can be reached through their website; vitalchek.com. An additional fee is charged by VitalChek for this service, and all major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®.

How to request a certified record by mail :
** The information on the ‘Application for Copy of a Vital Record’ form (see below) must be completed in order to make an application for a certified copy of a vital record (birth, death, marriage) registered with the Iowa Department of Public Health.
** In Iowa, registration of births, deaths and marriages began July 1, 1880. No records of events prior to July 1, 1880, are on file in this office.
** A fee of $20.00 is required for the record search and includes one copy.
** Each additional copy of the record is also $20.00.

** If the record is not located, the applicant shall receive a notification of the record search.
** The $20.00 fee shall be retained for the search.
** Upon completion of the application form with a notarized signature, attach a check or money order payable to the ‘Iowa Department of Public Health,
** ‘ a clear photocopy of the applicant’s current, government-issued photo identification, and submit to the address listed below.

How to request a certified record in person :
** Walk-in service is available for in-person requests at the state vital records office between the hours of 7:00 a.m. and 4:45 p.m., Monday through Friday, except for state-observed holidays.
** The office is located at the address listed below, just inside the main north entrance and main lobby.
** The Lucas building is the first state office building east of the state Capitol building, on the south side of Grand Avenue.

** Written applications must be satisfactorily completed and signed.
** Applicants must also provide valid, current government-issued photo identification.
** Same-day requests are accepted from 7:00 a.m. to 4:45 p.m. Monday through Friday and in-person only.
** Requests received by 2:00 p.m. Monday through Friday will be processed within the same day.
** Requests are processed in a minimum of 2 hours.
