Apply for NT Evidence Of Age Card
** This card is evidence of your age and is available to any Northern Territory resident aged 18 and over.

** The evidence of age card expires five years after issue.
** If you change your name or want a new photo you will need to apply for a new card.
** When changing your name or photo, you must surrender your old card to the motor vehicle registry (MVR) or a police station, and you must provide evidence of name change documents.
** Concession card holders are eligible for one free evidence of age card every five years.
** If you lose your card, it’s damaged or stolen, you can get a replacement card if you pay the evidence of age card fee.

** You will need evidence of identity and residency
How to apply :
You must follow these steps :
** Step 1 – Fill in the application for evidence of age card (193.1 kb)
** Step 2 – Provide supporting documents.
** Step 3 – Pay the evidence of age card fee.
** Step 4 – Submit your application to an MVR office by one of the methods below.
New MVR customers :
** You can submit your application form in person at a MVR office.
** You can pay your evidence of age card fee by cash, cheque, EFTPOS, BasicsCard, Visa or MasterCard.
Current MVR customers :
** If you are a current MVR customer and your photo was taken within the past five years, you can email or mail your application form.
Email :
** Send the completed application for evidence of age card form to mvr AT nt.gov.au. You can pay the fee using Visa or MasterCard.
Evidence of identity and residency:
Category A documents :
You can present any of the following :
** Australian birth certificate with the registration and certificate number
** Australian passport (current or expired up to two years)
** a current overseas passport (current or expired up to two years)
** Australian driver licence (current or expired up to two years)
** NT police and fire officer identity card
** Australian naturalisation or citizenship certificate
** Immigration department travel document (valid up to five years after issue).
Category B documents :
You can present any of the following :
** current Medicare card, NT Pensioner Concession Card, Department of Veterans’ Affairs entitlement card, NT seniors card or other concession card issued by the Australian Government
** current credit or debit card with signature and embossed name
** current student identity document issued by an Australian educational institution with photo or signature
** current Australian-issued security guard/crowd controller licence with photo
** current Australian-issued firearm licence with photo
** current Australian Defence Force identity card (excluding civilians)
** current evidence of age card or interstate driver licence
** overseas driver licence (current or expired up to two years)
** current NT Government employee identity card
** current NT work zone traffic management identity card
** current Larrakia Nation identity card
** current East Arnhem Regional Council identity card
** current Tangentyere Council identity card.
Other documents of residency :
If evidence of your current NT residential address is not shown on at least one of the documents listed in category A or B, you can submit one of the following :
** contract of purchase, current lease or rental document, receipt from an accommodation house or caravan park showing your residential address
** formal NT Government correspondence identifying you and your residential address dated within the last 12 months
** formal Australian Government correspondence identifying you and your residential address dated within the last 12 months
** council rate notice identifying you and your residential address
** financial institution document identifying you and your residential address dated within the last three months
** gas, electricity, water, telephone, pay TV, Austar, internet account identifying you and your residential address dated within the last three months
** Australian Taxation Office assessment showing current residential address
** document from an educational institution identifying you and your residential address
** completed parental or guardian evidence of residency verification form (205.1 kb) dated within the last three months.
