1. Application Form (Duly Filled) along with signed declaration of the Code of Medical Ethics attached with the application form.
2. A copy of qualification to be registered (degree/diploma/certificate) along with original copy.
3. Copies of transcripts/mark sheets of class 10, 12 and other qualification along wit original .
4. Two copies of recent passport size photograph.
5. A copy of CID / Passport for foreigners.
6. Registration fee (refer the table).
