How To Apply For Change of Name in COMELEC Voter ID?

** The Application Form for Change of Name, also known as CEF-1D, may also be obtained free-of-charge from the EO’s office.

** It is also available for download from this Website. It is to be printed on a long bond paper (preferably substance 20/70 gsm), after which the applicant shall manually fill-up the same in three (3) copies. There is no online version of CEF-1D which is currently available for online accomplishment.

Application Form :
Go to the official website, click on the “Voter Registration” tab in main menu

Then Select on the “Application Form – > Regular Forms” link, Application forms will be opened.

Click on ” Blue CEF-1D: Application for Change / Correction of Entries / Inclusion of Record in the Book of Voters and Reinstatement of Name in the List of Voters” the link Application form will be displayed as a PDF format.

These forms should be accomplished in three (3) copies as follows:
** Copy for the Election Officer;
** Copy for the Provincial File;
** Copy for the National Central File.

It is important to ensure that all copies have the same written information.

In support of the application, the applicant shall submit any original/certified true copy of the following :
** Certification by the solemnizing officer;
** Marriage contract or court order with certificate of finality;
** Order by the Civil Registrar or Consul General, as the case may be.

Note :
This may also be availed of in cases where the marriage of a registered voter has been annulled or declared as void by competent court. In this case, the affected voter may request to revert back to her maiden surname and for the change of signature. The applicant shall attach a certified true copy of the Court’s final ORDER/DECISION, annulling the marriage or declaring the marriage a nullity.

What are the Procedures for Filing Applications?

Any registered voter whose name has been changed by reason of marriage or court order or by order of the Civil Registrar or Consul General may file an application for change of name by personally accomplishing three (3) copies of the prescribed application form (CEF-1D – Application for Change). It is important that the EO shall check the box corresponding to the appropriate type of application.

The EO shall verify the record of the applicant in the database. If his/her name is in the database, he/she shall be instructed to proceed to the VRM Operator.

The VRM Operator shall, using the old application form number :
** Select File -> Other Application -> List of Records;
** Type the last name and/or first name and/or maternal name in the space provided and then click SEARCH button;

** Right-click the record of the applicant and select CORRECTION OF ENTRIES from the list of application type;

** Edit the entries for correction; and
** Save the record.
** Once the process is completed, the EO and VRM Operator shall then perform the procedures stated in Section 12, Paragraph (h) last sentence and Paragraph (i) hereof.

Note :
After the Board approval, the EO shall consolidate/merge the updated data.

Functions of COMELEC

The Commission on Elections (COMELEC) of the Philippines is an independent constitutional body responsible for the conduct of elections in the country. It is one of the three constitutional commissions, along with the Civil Service Commission and the Supreme Court.

The COMELEC’s functions are as follows:
** Enforce and administer all laws and regulations relative to the conduct of an election, plebiscite, initiative, referendum, and recall. This includes setting the dates of elections, registering voters, printing ballots, and canvassing votes.
** Exercise exclusive original jurisdiction over all contests relating to the elections, returns, and qualifications of all elective and appointive officials. This means that the COMELEC is the only body that can hear and decide cases involving election disputes.
** Appoint, remove, and discipline all election officials and employees. This includes the chairman and members of the COMELEC, as well as the provincial, city, and municipal election officers.
** Initiate and undertake measures to educate the public on the electoral process. This includes conducting voter education campaigns and publishing educational materials on elections.
