ICA Loss/Damage of Long Term Visit Pass Card

If you have lost your valid Long-Term Visit Pass (LTVP) card or your LTVP card has been damaged or defaced, you should apply for a replacement in person within seven days.

If you subsequently recover your LTVP card after reporting it lost to ICA, you must surrender the recovered card to ICA within seven days. You should also apply for a replacement card to update your particulars.

Documents Required

To replace a lost or damaged LTVP card, you will need :
** Your recent passport-sized colour photograph taken within the last three months. Please click here for photo guidelines.
** A travel document that is valid for at least six months
** An original police report (not a photocopy) or a statutory declaration explaining that the card has been lost, if applicable.

Procedure

To apply for a replacement card, go to :

10 Kallang Road, Level 4 (next to Lavender MRT station)
Singapore 208718

This service is available from 8.00 am to 4.00 pm on Mondays to Fridays (except public holidays). Please note that your local sponsor may also need to be present.

Fees

You will need to pay the following fees :
Replacement fees for a lost LTVP card
** S$100 for first loss
** S$300 for second and subsequent losses

Replacement fee for a damaged LTVP card, or replacement due to change of personal particulars
** S$60

Collection

You will need to make an appointment to collect your replacement LTVP card at least three working days in advance using the e-Appointment .

Self-Service at iCollect

iCollect is a self-service machine housed in the eLobby on the ground floor of the ICA Building. Collection has to be in person.
You are not eligible to use this service if you :
** Have poor-quality fingerprints due to ageing or excessively wet or dry thumbs and fingers
** Do not have two sets of fingerprints enrolled in ICA’s database
** Are aged 16 and below.

You are not eligible to use this service if you :
** Have poor-quality fingerprints due to ageing or excessively wet or dry thumbs and fingers
** Do not have two sets of fingerprints enrolled in ICA’s database
** Are aged 16 and below.

Change of Residential Address for Long-Term Visit Pass Holder

You are required to notify ICA if there is any change in your place of residence within 14 days of the change. You can report a change of address online through your local sponsor (spouse, parent or child), using the eService.

You and your sponsor will be contacted by ICA for additional documents or to arrange an interview, if required.

If you are unable to report your change of address online, you and your local sponsor can report the change in person at Visitor Services Centre (VSC) :

Requirements

The following documents are required for reporting change of address at the VSC :
** Duly completed and signed Change of Address Form*
** Your passport biodata page
** Your Long-Term Visit Pass card
** Your sponsor’s Identity Card.
