Mohap Modify Birth Certificate Details

Service description – Changing or modifying the birth certificate details, and the issuance of the new original birth certificate, and return the old original birth certificate.
Conditions & Requirements
1. The presence of the concerned person or his representative under an official proxy
2. Bringing the original documents
Changing the name of a new child registered in the registration record.
3. A letter from the Department of Nationality addressed to the preventive medical center of non-objection of changing the name.
Changing the name of the child for the residents:
1. Non Objection Letter from the consulate of the country to which the child belongs, indicating the change of the name of the child.
2. The original certificate issued by the preventive medicine department.
3. Visiting the same preventive medical center from which the original birth certificate was issued
Required Documents
1. Original passports, Emirates ID and residence card in the case of non-nationals, and copies thereof
2. The original marriage contract duly attested in case the marriage contract is concluded outside the s=State
3. Original birth certificates. In the event of loss of the certificate, you shall provide a letter from the police station
4. In case of changing the name, you shall provide a letter from the consulate and a letter from the court to make the required change.
Procedures & Steps
1. Visiting the Preventive Medical Center with the required documents (the same preventive medical center that issued the original birth certificate first time)
2. Inserting the application in the electronic system and paying the fees.
3. Verifying the documents and original copies, and issuing the birth certificate
4. Receiving the certificate by the father, or his representative
