Procedure For Lost/Damaged Card For GCC Citizens :
This service enables (GCC) citizens to report and issue a lost / damaged card

Eligibility or Criteria :
** Concerned must attend in person or his / her legal guardian or legal representative
** Lost / damaged card form must be signed
** New card will be issued after (60 days ) of report
** (Certificate good for (60days ) can be issued upon request for (1 KD
Required Documents :
** original passport , and a copy of it
** Original civil id card for (damaged card) only
** 2 recent personal photos according to specifications over 5 years old
** Blood type, If not registered in old card
** Signed photo form
** Original power of attorney and original id of the empowered
Procedures :
** Visit PACI head office – south surra – 1st floor – citizens and GCC registration office – working hours 8am to 1pm
