PAN Foundation COVID-19 Financial Support Fund
To help affected patients cope with unprecedented financial stress relating to the COVID-19 outbreak, also known as corona virus, we’re offering direct assistance of $300 on a pre-paid debit card for individuals who have been diagnosed with COVID-19 or directed to self-quarantine due to COVID-19.

Eligible patients can use the financial support for treatment expenses for COVID-19, over-the-counter and prescription medications, medical equipment and supplies, and groceries and meals, including delivery services.
Program Status
Open – We are currently accepting applications from individuals.
Eligibility Qualifications
** The patient must be diagnosed with COVID-19 or directed to self-quarantine by their healthcare provider or government agency.
** The patient must also have cancer or a chronic or rare disease.
** The patient must have Medicare health insurance.
** The patient’s income must fall at or below 400% of the Federal Poverty Level.
** The patient must reside and receive treatment in the United States or U.S. territories. (U.S. citizenship is not a requirement.)

How Does it Work?
** If you’re approved for a grant, you will receive a Visa debit card pre-loaded with $300 in the mail that you can use to pay for covered services.
** Your debit card will be active for 12 months or until you use the entire amount—whichever comes first.
** You will receive the debit card in the mail within 10-15 days. When you receive the card, there are two ways to activate it.
** Email us at activate [AT] panfoundation.org with your first and last name, that you’re receiving assistance for the COVID-19 Financial Support Fund, and the last 4 digits of the card.
** We will send you a confirmation email once your card has been activated and is ready for use.

You can use your debit card to pay for the following items or services

** Over-the-counter medications and prescription medications, including delivery services
** Medical equipment and supplies
** Transportation to treatment or pharmacies

Please note that we reserve the right to terminate your grant if a transaction has not been made on your card in 90 days or if you use the card for non-covered services or items.
2. If I spend the entire $300 and still need help, can I apply again?
At this time, individuals approved for a grant are only eligible to receive one grant from the COVID-19 Financial Support Fund so that we can help the most people possible. Thank you for your understanding.
3. How do I check my balance?
You can check your available balance, review your transaction history and download monthly card statements any time at www.pexcardholder.com.
4. What should I do if I lose the debit card?
If you lose your debit card, you can contact us at 1-866-316-7263, or PEX (the card distributor) at 1-866-685-0898.
