** Central Medical Committee Decision.
** Special Military Committee Decision.
** Written request.
** Certified copy of the Family Booklet.

3-10 regular workdays.
Requested Fees :
As per the Stamps Fees Law.
Place of service provision :
At the center : Civil and Military Retirement Committees.
Work Procedures :
** Ensuring that all required documents are complete.
** Providing the applicant with a card to schedule an appointment.
** Opening an application document.
** Regulating the decision and having it signed by the retirement committee and relevant authorities.
** Notifying the applicant of the decision during the scheduled appointment.
** Sending the application to the Retirement Directorate for payment.

Required Documents :
** The committee’s decision to approve the amendment.
** The average time needed for the completion of this service :
** One hour after the announcement of the committee’s decision and the preparation of the official letter of correction.
Place of service provision :
** At the center; The Retirement and Compensations Directorate or the Salaries Division.
** At financial centers in governorates and province s; can be sent by fax to financial centers as requested.
Work Procedures :
** Amending the name on automated forms and send it to data entry staff.
** Preparing and directing an official letter to related authorities to include the correct name as requested.
