Process for obtaining a certificate of Occupational therapy :

Submitting an application for a certificate :
** The request must be sent, along with the required documents, by registered mail to the Medical Professions Licensing Department, Ministry of Health, as detail at “Service Details” below.

** Assessment of the applicant’s eligibility to take the examination
** After receipt of the application, the applicant’s eligibility to sit the examination is assessed by the Division of Medical Professions. A notification will be sent to eligible applicants summonsing them to an examination, to the address held in Ministry records.
Government Examination :
** The examination is theoretical.
** Those who are eligible to take the examination will be sent an invitation to the examination, including the location and the examination procedures, as per the address recorded in the Ministry.
** The examinations are held twice a year (winter examination and summer examination) and are published on the website .
** The examination is held in the Hebrew, Arabic or English language.

Reception Area :
The request must be sent by registered mail to the Medical Professions Licensing Department, Ministry of Health, 39 Yirmiyahu St., P.O.Box 1176, Jerusalem 9101002.

Requirements prior to receiving Service :
** Application form to take the examination in occupational therapy .
** Questionnaire for health care professionals.
** 2 passport photographs.
** Photocopy of your identification card, including the attachment showing your address.
** Final diploma in occupational therapy, or alternatively certification from the university that you have completed your studies and have fulfilled all obligations to the university and are eligible for a degree in this profession, which will be awarded at a certain time.
** Official confirmation of the studies start and completion
** ranslation and validation of documents and certificates
** riginal certificates must not be sent to the medical professions licensing department. Instead, only copies must be sent.
** certified copies of documents should be certified by a licensed notary in Israel. The original notarized certification (with the red ribbon) should be sent to the Division, as well as an additional copy of it (a total of two copies).
** Documents in languages other than Hebrew and Arabic must be translated into Hebrew by a qualified translator in Israel. A copy of the original document and its translation must be submitted.
